Relationship Between Iron Deficiency, Iron Deficiency Anemia And Helicobacter Pylori Infection In Children

Objective To evaluate the relationship between iron deficiency,iron deficiency anemia(IDA)and Helicobacter pylori(HP)infection in children.Methods There were 2000 children for this study,among whom,899 cases were from the nursery(GroupⅠ),1101 cases were hospitalized children who did not have the hemorrhagic diseases of digestive system and the diseases of blood system(GroupⅡ).The 3 ml fasting vein blood was taken from each case.The 0.5 ml blood was used for blood routine exam,the rest was used to test HP-IgG antibody,serum iron and serum ferritin.We selected out 58 cases from GroupⅠwho were HP-IgG positive and IDA.The 58 cases were divide into two groups(GroupⅠA and GroupⅠB).GroupⅠA was treated by anti-HP therapy in conjunction with oral ferrous sulfate syrup.GroupⅠB was treated by ferrous sulfate syrup only.The haemoglobin and serum ferritin were measured after 8 weeks.Results①Among the 899 children from the nursery(GroupⅠ),279 cases were HP-IgG positive(31%),99 cases were diagnosed as IDA(11%).Among 99 IDA cases,58 cases were HP-IgG positive,the ratio of IDA in HP-IgG positive case was 20.8%(58/279),41 cases were HP-IgG negative,the ratio of IDA in HP-IgG negative case was 6.7%(41/620)[x~2=39.46,P＜0.01].The ratio of liP infection in IDA case was 58.6%(58/99).Among the 800 non-IDA children,221 cases were HP-IgG positive,the ratio of HP infection in non-IDA children was 27.6%(221/800)[P＜0.05].②Among the 1101 hospitalized children(GroupⅡ),149 cases were HP-IgG positive(13.5%),361 cases were diagnosed as IDA.Among 361 IDA cases,80 cases were HP-IgG positive,the ratio of IDA in HP-IgG positive case was 53.7% (80/149),281 cases were HP-IgG negative,the ratio of IDA in HP-IgG negative children was 29.5%(281/952)[x~2=34.16,P＜0.01].The ratio of liP infection in IDA case was 22.2%(80/361).Among the 740 non-IDA children,69 cases were HP-IgG positive,the ratio of HP infection in non-IDA children was 9.3%(69/740)[P＜0.05].③58 cases,who were HP-IgG positive and IDA from GroupⅠ,were divide into two groups(GroupⅠA and GroupⅠB).GroupⅠA had 30 cases,and GroupⅠB had 28 cases.Before and after treatment,the Hb and serum ferritin were 107.3±4.2g/L and 14.19±0.98ug/L,117.4±4.7g/L and 19.54±3.13 ug/L respectively in GroupⅠA[P＜0.05].Before and after treatment,the Hb and serum ferritin were 107.5±1.9g/L and 14.07±1.09 ug/L,107.9±1.7g/L and 13.65±2.84 ug/L respectively in GroupⅠB[P＞0.05].Conclusions The ratio of HP infection in IDA case is higher than that in non-IDA children.And the ratio of IDA in HP-IgG positive case is also higher than that in HP-IgG negative children.So there is correlation between HP infection and IDA in children,HP infection is a risk factor of IDA in children.
