ObjectiveThe maxillofacial traumas and fractures in the elderly were increasing rapidly in recent years, following the development of the highway and transportation tools and the aging of population. The maxillofacial bones were injured easily because of exposed in the surface of the body and rarefaction of the bone. The maxillofacial fractures in the senile patients can cause them to the facial deformity, the functional disturbance and the mental disorder.The purpose of the operation to the senile patients with maxillofacial fractures were restore of the masticatory function and the prosopo-outline, to avoid the happening of psychologic obstacle. The hypofunction of the organs and the multisystem diseases in the aged people raised the complexity and risk of the fractures operation.The purpose of this study was: to evaluate the operative risk factors and level in the senile patients with maxillofacial fractures allroundly, and took some precaution measures to decrease the risk rate of the operation.Material and MethodsTo retrospective analyze 42 cases with maxillofacial fractures in the elderly. Data regarding pattern of fracture, systemic disease, treatment and outcome were reviewed. Make a evaluation for the functional level of the cardia, pulmo, liver, kidney and brain. We divided the operative risk rate into four grades: low, middling, high and very high. To analyze the factors for the operative risk and the precaution measures. According to the risk grades, we took some differential therapeutic measure for these patients.ResultsThere were 14(33.3%) patients with heart diseases, 12(28.3%) patients with hypertension, 7(16.7%) patients with diabetes, 6(14.3%) patients with Diseases of respiratory system, 2 patients with Hypoproteinemia and 1 patient with Hypopotassaemia.The result of the evaluation to the operative risk rate was low have 7 cases, middling have 13 cases, high have 18 cases and very high have 4 cases.There were 26(61.9%) patients be taken the operation of open reduction and internal fixation, 9(21.4%) patients be taken the intermaxillary fixation with intermaxillary fixation screw or arch bars. The curative effect of these patients was satisfied.ConclusionIt was very important that we must make a risk assessment for the elderly patients with maxillofacial fractures in preoperative. It can avoid some severe operation complications. The most of the ederly patients can tolerate the maxillofacial fractures' operations and get a satisfying results.
