Disposition Rule Of Syndrome Differentiation In Traditional Chinese Medicine In Chronic Pelvic Inflammatory Disease In Guangzhou Area

Background and purposes of the research:Pelvic inflammatory disease(PID) belongs to infaction diseases on upper tractus genital is. It is one of the most commonly and frequently encountered diseases in gynecology, which may cause serious sequelae like ectopic pregnancy and sterility of Fallopian tube orgin , hence it will effect women's physical and mental well being greatly. Modern medicine treats it with antibiotics and expectant treatment mostly. Antibiotic treatments to PID that American CDC recommended in 2006 involved oral antibiotic project and external-used drug project. But as PID has special pathological changes, it' s hard for antibiotics to be effective in the pelvic cavity partially, so many patients' symptoms can be alleviated when taking medicine and recur without it.Treatment based on disease identification and macro-regulation in theory of TCM specialize in diseases which is incurable and easily recur, such as Chronic PID. Though there is not a correspondent name of it in acient books of TCM, according to its clinical manifestation, doctors nowadays put it into categories of "furen futong","irregular menstruation","infertility" or "diseases of leucorrhea" ,ect. Therefore, there is not a unanimous taxonomy. Without epidemiology research data for each pattern of Chronic PID, the standardization of treatment based on categorical identification in TCM theory is obstructed to a certain degree. And further influence the improvement of the overall effect of TCM clinic. This attention is preliminarily concentrated on the distribution of Chronic PID in Gguangzhong Area in TCM categories, and trying to conduct useful probes for the standardized treatment based on disease identification of Chronic PID, using statistical methods of epidemiology combined with mathematics. It has a significance for a great range of standardized classification work to Chronic PID in the future, and carry out basic work on follow-up researches.Object of study:200 cases with Chronic PID that meet the standard are patients who visisted gynaecological clinic or had been in the inpatient department of Hospital of TCM of Guangzhou from March 2007 to March 2008.Methods:Design "Chronic PID information collecting form by TCM Syndrome four consultation" on the base of extensive access to documents and combining expert advices. Investigate the 200 cases factually at the scene by cross-sectional survey. Input and verify the information and establish a database using EpiData 3. 0 system. Statistic further the information input by descriptive analysis, cluster analysis, and correlation analysis, using SPSS 14. 0 system.Results:According to the overall basic syndrome of PID, the frequency analysis shows that, the top fore is: humid heat(67 cases, 33. 5%), humid caked with heat (37 cases, 18.5%), humidity originated from spleen deficiency(28 cases, 14%), Qizhixueyu(26 cases, 13%). Implement reduced-order processing by principal component analysis after categorizing successfully for the main symptoms indicators. And found that four- type of cluster showing a good clustering effect. Here follows symptoms of each syndrome:1. Humid heat: Abdominal pain fearing passion, low fever, yellow and smelly leucorrhea, thirsty and bitter taste, dry stool, yellow and less piss, Red tongue with dry and yellow,greasy and thick fur, Waterloo and quick pulse.2. Qi-stagnancy and Blood-stasia with Humid: tingles (sharp pain) in the lower abdomen, abdomen-ache,breasts-pain and lots of blood clots in menstrual period and agitation in front of it. Sighs for depressed thoughts, pains in the chest, petechia on the tongue, acerbity pulse.3. Humidity originated from spleen deficiency: dull pains in the lower abdomen, lethargy and weakness, dizziness and humming ears, tasteless and little appetite, lean leucorrhea, lots of menstruation with light color, lean stool, litht-colored and increased tongue, dents on the edge of the tongue, narrow and weak pulse.4. Kidney-asthenia and blood stasis :soft pain or cold pain in the lower abdomen, waist and knea sour, limb cold, leukorrhagia, dark tongue , has petechia or ecchymosia on the tongue, white coat, string pusle.There were 114 cases of humid heat type, takes 57%, which is the most basic one in chronic PID in Guangzhou area. But there are also some cases of asthenia type , such as Spleen-asthenia, Kidney-asthenia. This is a asthenia-evil disease, so except clering away the heat evil and promiting diuresia , we shuold nourinshing the defficiency. In all, the basic pathogenesis-shi re yu xu-of PID was well presented in the upper four syndromes. It can provide a suitable program and simplify for clinic therapy.
