Objective: To understand health services need, utilization and medical costs of HIV infected persons for the basis for the health sector making the policies and measures in Urumqi. Methods: By personal interviews for 88 patients of Urumqi collected information of their health services need, utilization and medical costs. Results: In 88 HIV infected persons 56 cases were male, 32 cases female; 9 cases were Han nationality, 77 cases Uygur, 2 cases Hui nationality, the average age was 32-year-old.Two weeks prevalence of patients was 81.8%, two weeks sick days was 6.10 days, two weeks bedridden rate was 58.0%, two weeks bedridden days was 2.67 days. Chronic illness prevalence was 47.7%. Persons with physician contact within two weeks were 88.6%; persons without physician contact within two weeks were 41.5%. 58.1% did not treat because of economic difficulties. Persons with hospital admission within twelve months were 30.7%. An average volume hospital day was 21.2 days. Persons without hospital admission within twelve months was 35.7%, the main reason was economic difficulties (74.0%). 21 patients were carrying out free antiretroviral treatment which our country provided. 71.4% was 90% to 100% adhere to medication, 23.8% was 50%~90% medication, 4.8% <50% intermittent medication. The main reasons for intermittent medication were opportunistic infections and adverse reactions. 39 patients which could share with free antiretroviral treatment which our country provided, 33 patients untreated for a ratio of 58.8%. 57.1% have not been anti-virus treated because of financial difficulties. The cost out-patient of HIV infected persons was 106 Yuan (median 67 Yuan). The cost of self-health care in two weeks was 75 Yuan (median 40 Yuan). The cost of hospitalization was 6,475 Yuan (median 3,171 Yuan), the average total cost of medical care was 3,536 Yuan (median 1,000 Yuan) in the past year. Conclusions: The need and utilization of health services were large; the health sector should make the corresponding measures according to the characteristics of health services. Should be vigorously publicize and implement the Four Frees and One Care Policy, and establish medical assistance mechanisms to meet utilization of health services of the patients.
