Comparative Analysis Of Transabdominal Reperitonea1 Laparoscopic Inguinal Herniorrhaphy And Tension-free Herniorrhaphy

Objective To study the superiority and efficiency of transabdominal preperitonea1 laparoscopic inguinal herniorrhaphy.Methods 60 cases of laparoscopic inguinal hernioplasty with transabdominal preperitoneal(TAPP) and 108 cases of open inguinal hernioplasty (OIHP) with tension - free were performed from October 2004 to December 2006 .Perioperative data and follow-up results were collected and compared in two groups.Results 60 cases were received transabdominal preperitonea1 laparoscopic inguinal hernia repair successfully,operation time of TAPP group was 34～75min, average 49.080±10.887 min. OIHP group's operation time was 30～71min, average 47.940±10.184 min, there were no significantly different between two groups(P=0.504,P >0.05);hospital stay of TAPP group was 2～5d, average 2.930±0.989d. OIHP group's hospital stay was 3～8d, average4.400±1.238 d, there were significantly different between two groups(P=0.000,P<0.001); Hospitalization costs of TAPP group was 6220～8706$ ,average 7306.57±621.01 $ , OIHP group's costs was 3659～6660 $ ,average 4921.82±707.82$,The costs of hospitalization in TAPP group was higher than the OIHP group,There were significantly different between two groups(t=-21.84,P=0.000,P <0.001);There were no significantly different between two groups about occurrence rate of complications; Follow-up 6～48 months, there were no recurrent patient in the TAPP group,1 patient was relapsed in those OIHP group.Conclusion Transabdominal preperitonea1 laparoscopic inguinal herniorrhaphy is a safe,reliable , efficacy with advantages of mini-invasion , better recovery, simple procedures,shorter hospital stay , low relapse rate and and is especially suitable for bilateral and recurrent hernia. It the deserves to be expanded.
