| Objective: The key point of curing chronic gastritis is to make a definite diagnosis of Helicobacter pylori infestation.So,this study through magnifying endoscopy to view gastrer mucosa appearance to judgement Helicobacter pylori infestation.To evaluate the efficacy of magnifying endoscopy in diagnosis of Helicobacter pylori associativity gastritis.Methods: The fine structure of gastric mucosa in 200 patients with different types of chronic gastritis was studied using Olympus GIF-Q240Z magnifying endoscopy. Biopsies were taken from the magnifified sites. The fine structure of gastric mucosa were compared with the results of rapid urease test.Using chi-squared test to carry out statistics analysis on the basis of result.Results: Collecting venules in the anterior wall at lower part of gastric corpus were subgrouped into types Regular,Irregular and Disappeared . In the gastric antrum, the respective Hp inffection rate of these three types of gastric pits was 9.5%(4/42) ; 86.67%(52/60); 87.76%(86/98). Among them, Hp infection rate in type Regularwas significantly lower than that in the other two types(P< 0. 0l) . Gastric pits of antrum were classified as types A(punctiform),B (short rod), C (branch), D (reticular), and E (villus). The gastric mucosa with type C pits had a higher infection rate than that with type B pits (P<0.01). The rest type have no statistic difference(P>0.05). The 5 gastric pit patterns, pattern A is normal gaster mucosa;CSG reside in pattern B and pattern C,the rate is 18.4%,72.6%.CAG reside in pattern C,pattern D and pattern E,the rate is 20.3%,83.7%,90.5%,the rate of intestinal metaplasia is 8.2%,57.6%,93.4%.Conclusion: It is clear that the fine structure of gaster mucosa to change by using magnifying endoscopy. The microstructual changes of gastric pit and collecting vein can reflect the severity of gastric disease . Helicobacter pylori infestation are to show change of pattern B, pattern C, pattern D, pattern E of gastric pit and pattern I, pattern D of collecting vein .Pattern E of gastric pit is the mark appearance of intestinal metaplasia .Our results indicated that magnifying endoscopy might be potentially valuable in the diagnosis of Hp infection of gastric mucosa. |
