The Discussion Of The Significance Of Renal Biopsy In Diagnosing The Asymptomatic Renal Glomerular Disease

The physiologic functions of the kidney are excreteing the metabolin,controlling the balance of electrolytes and acid-base,and maintaining the stabilization of the internal environment of the body by making different component of uria.The powerful controlling ability of the kindey depends on the filtered area of the glomerular,sufficient blood supplying,filtered ability and the reabsorbtion and excretion of the renal tubule. Proteinuria and hematura are the major clinical manifestation of the renal disease ,they are also the major reason of why partial of the person who have the asymptom glomerular disease go to see the doctor. Bcause urinary disease and other system disease can also have these symptoms, so the specificity of the diagnosing the renal golmerular disease by them are very low.In the meantime, because the powerful compensation ability of the kidney, there are no other symptoms in the early period of the damaging.In the conventional idea, the damaging location and degree of the kidney by the changing of the laboratory examination,so many doctors and patients ignored the damage of the damage of the renal parenchyma. As a result, the diagnosis and treatment are delayed, and the desease was advanced. In order to get a direct,effective method to diagnosing the damage of kidney, our research combined the imageologic method,surgical technology and pathologic diagnosing, established a percutaneous renal biopsy method under the guiding of the ultrasound.The principal fo the experiment: We combining the imageologic method,surgical technology and pathologic diagnosis.In the mediation of the ultrasound, we puting the puncture needle to the location where we want to by visual,getting the tissue and diagnosing by the pathology.Experimental procedure: The patients get the prone position, exposing the back. we definited the location of the kidney and the puncture point by the ultrasound. After the rontine sterilizing the operational area, under the guiding of ultrasound,we get the renal tissue by the percutaneous puncture. The tissue were making to the pathological section, and we examined the pathological section by the optical microscope,fluoresence microscope and electro microscope. The pathologic diagnosis according to " the revise of histologic grouping of renal glomerular disease "(WHO1982 and 1995)".In our research, 120 patients whose urinalysis was normal were enrolled.The results implied that the pathologic change of the patients can divide to 7 type:mesangial proliferative nephritis,IgA nephropathy,glomerulosclerosis,local hyperplasia nephritis,local segmental hyperplasia nephritis,I type membranoproliferative nephritis,blood capillary;micrangium hyperplasia nephritis and normal tissue.The technique of percutaneous renal biopsy have higher sensibility and specificity, The results are direct-viewing,and easy to analysis. It is a good index for diagnosing and antidiastoling the renal damage.
