Study On Recurrence Factors Of Postoperative Differentiated Carcinoma Of Thyroid

Objective Although the most thyroid cancers are well-differentiated low malignant neoplasms and their prognoses are relatively good generally if they were treated routinely,there still a few cases of this tumor recurred.There are many controversies concerning the treatment to patients with differentiated thyroid carcinoma(DTC), focus on appropriate resection extent of DTC ,indications for cervical lymph node dissection,et al. This study was designed to sum up and analyze the cause,treatment ,and prognosis of these cases to seek for the cause and the proper treatment method of the recurrent cases.Methods Thirty - three postoperative recurrence cases of differentiated thyroid carcinoma of three hospitial were analyzed retrospectively,and observe the prognosis - related factors. Recurrents rate of patients mainly received local mass excision with or without cervical lymph node resection with higher than that of patients mainly received unilateral lobectomy plus isthmectomy with or without cervical lymph node dissection(P > 0.05) .Results There was a close relationship between postoperative recurrence and the condition of tumor ,lymph node , metastasis as well as operative manner. Conclusions Operation manner should be selected depending on the condition of tumor. Postoperative recurrence is the main reason of therapeutic failure. The operational methods of differentiated thyroid carcinoma should select resection of pathological lobe and isthmus and subtotal resection of contralateral lobe, or total thyroidectomy according to the pathological type, clinical stage and should perform radical dissection of neck lymph nodes if there was neck lymph node metastasis. The prognosis of differentiated thyroid carcinoma is closely related with age, pathological type, clinical stage radiotherapy or take thyroxine long - termly after operation.
