?Objective?Retrospective analysis was conducted to study the related factors that may affect postoperative recurrence of differentiated thyroid carcinoma.And the risk factors were gradually screened out to provide certain clinical references for intraoperative management and postoperative follow-up of patients with high risk of postoperative recurrence.?Method?A total of 62 patients in Fujian Provincial Hospital who had undergone radical thyroidectomy for thyroid cancer for more than 3 months during the period of operation from September 2012 to December 2018,and whose postoperative pathological diagnosis was differentiated thyroid cancer were included in the recurrence group.A total of 159 patients without tumor recurrence were selected as the control group during the above period.Two groups of patients were retrospectively analyzed,observing indexes of patients with gender,age,primary tumor size,tumor relationship with capsule,the number of positive neck lymph nodes(including both central and lateral neck areas),TNM stages,the presence of merger with chronic lymphocytic thyroiditis,presence of nodular goiter,mutated gene of BRAF V600 E,Ki67,CK19,Galactin-3,MC,CD56,researched on the relationship between those indexes and tumor recurrence.SPSS 19 statistical software was used to initially screen the variables that may be related through univariate analysis,and then independent risk factors affecting postoperative recurrence were screened through multivariate regression analysis.?Result?Multiple central tumors(P=0.014,OR=2.442),invasion of the capsule(P=0.044,OR=3.006),lymph node metastasis in the central region(P=0.033,OR=2.487),single tumor with the maximum diameter of > 17mm(P=0.031,OR=3.274),all of which showed that P<0.05,suggesting that the above four observation indicators were independent risk factors for postoperative recurrence of differentiated thyroid cancer.And the rest of the observation indicators such as whether tiny cancer,extrathyroidal extension,lateral neck area with and without lymph node metastasis and level of Ki67,expression of CK19,expression of Galectin-3,expression of MC,level of TSH,age(divided into three groups according to the age of 30 and 55 years),according to the statistical results,were denied as independent risk factors for recurrence of differentiated thyroid carcinoma(P > 0.05).?Conclusion?Multicentric tumor,invasion of capsule,lymph node metastasis in the central region,and the maximum diameter of single tumor(>17mm),were the independent risk factors influencing the postoperative recurrence of thyroid carcinoma according to this research.And the postoperative recurrence often occurred in the lateral neck area.In the clinical treatment,postoperative review and follow-up should be strengthened for patients with high risk of recurrence,so as to detect recurrence lesions early and make treatment.
