Objective:To evaluate the effects of carrying on acute hypervolemic hemodilution(AHH) and AHH combine with controlled hypotension(CH) and comparing with not taking any measure on coagulated function in spiral surgery and observe hemodynamic changes in Patients.Method:36 patients were randomly divided into 3 groups.GroupⅠwas Controlled Group,GroupⅡwas received hyper-volemic hemodilution in preoperation, and GroupⅢwas received hypervolemic hemodilution combined with controlled hypotension.Blood loss and blood transfusion were recorded respectively.MAP,CVP was monitored and recorded at before AHH,10min after AHH, 40min after operation time points.Coagulated function was measured before AHH, 10min after AHH,40min after operation and 1 day after operation.Result:GroupⅢhad less blood loss.As for blood transfusion volume, the trend was GroupⅠ>GroupⅡ>GroupⅢ.Hemodynamic changes were stable,MAP were maintained in patients except in GroupⅢwhich were deliberately hypotension but stable and under controlled; CVP were no significant different among groups. There was no significant difference of coagulation parameters among groups,PLT and PT were decreased but among normal extent, and 1 day after operation there were no significant difference among groups, APTT and FIB did not changed at different time point and had no significant difference among groups. Conclusion:AHH and AHH combined with CH were useful strategies for decreasing allogeneic blood transfusion, and these strategies did not significant influence on coagulation changes.
