Primary Discussion On The New Way Of Metabolic Syndrome Of Physical Examination

The concept of metablolic syndrome (MS), a cluster of metabolic alterations associated with cardiovascular disease, stroke, type2 diabetes mellitus and so on. Insulin resistance has the potential to contribute to some of the key questions. The clinical mark of MS is associated with IR,high TG, hypertension and abdominal obesity. With the development of economics and improvement of life style, the prevalence ratio of MS increases. Consequently, the treatment and prevention of this disease has become a public health priority. Data analysis through the blood glucose,blood lipid, waist circumference (WC) and blood pressure identified the disease. It will take a lot of time, not a good way for physical examination of MS.So we decide to take the anthropometric measure of fat mass as a new way on physical examination of MS.Methods:We chose 1282 subjects (all older than 20) who visited our department from April 2007 to December 2007. Blood pressure, triglyceride (TG), high-density lipoprotein cholesteol (HDL-C), blood glucose, WC, arm circumference (AC), leg circumference (LC), height (H), sagittal abdominal diameter (SAD), transverse abdominal diameter (TAD) were collected and anthropometric of fat mass index was calculated.The mets was defined according to IDP in April, 2005. To divide the subjects into three parts,one part was case group, other was control group, another was middle group. Comparing the difference of indexes (W/H, WC/AC+LC and SAD/TAD) in three groups.Result:1,There were significance difference of W/H, WC(/AC+LC),SAD/TAD and BMI between the case group and control group (p<0.001). There were also significance difference of W/H, WC/(AC+LC),SAD/TAD and BMI between middle group and control group (P<0.001).2,The prevalence ratio of MS tended to increase with age among women over 50 years.There had significance difference between the groups (P<0.05). The prevalence ratio of MS tended to decrease with age among men over 70 years. There had significance difference between the two groups (P<0.05). The prevalence ratio of MS in women was higher than in men among people over 50 years.There had significance difference between two groups (P<0.05).Conclusion:1,The way of anthropometric measure of fat mass can be used as a new way on physical examination of MS. The mechanism is:chosing the real WC increased ones with W/H, chosing real abdominal obesity ones, excluding overall obesity ones with WC/AC+LC, chosing the ones who are intraabominal fat, excluding ones subcutaneous fat, at last we will find out the patients of MS.2,The prevalence ratio of MS tended to incease with age among women over 50 years, much higher than man at the same age, it may relate to the change of estrogen. The prevalence ratio of MS tended to decease with age among men over 70 years. One of the reasons is that MS patients will die at young age. The other is the number of the study sample is too small.3,We should pay more attention to middle group, because this group can cause big social problem.4,This method can not be used in physical examination of MS immediately. Firstly, the number of sample is too small, secondly, location limitation,thirdly, the best cut-off points for waist is unknown.In this study, the way of anthropometric measure of fat mass can be applied as a new way on physical examination of MS. It is reasonable, fast, easy and economic. Specific criteria for MS among adolescents need to further research. The relationship between estrogen and MS also need to further reseach. People should pay more attension to the MS of physical examination in middle group.
