Objective To study the expression and significance of TGF-β3 and P21 and their roles in the mechanism and process of extra-mammary Paget's disease(EMPD).Methods 12 EMPD and 12 normal skin specimens received immunohistologic staining to observe the expressions of TGF-?3 and P21 in different tissues. Those specimens were classified into 2 groups according to the pathological classification criteria proposed by Wilkinson and Brown, which non-invasive EMPD and invasive EMPD, and compared with the expression of TGF-β3 and P21 as well as prognosis between these two groups.Results TGF-β3 was expressed in normal skin and EMPD tissue, the expression in normal skin was lower but that in the latter was higher, and the difference between these two groups was significant(x2=14.34, p<0.01), TGF-?3 expressions in noninvasive and invasive EMPD were significantly different with a higher expression in invasive EMPD(x2=5.21,P<0.05),that also happened between the noninvasive and invasive EMPD groups (x2=11.81, P<0.01),and the higher expressions occurred in the normal skin and noninvasive groups (x2=6.77,P<0.01). In EMPD group,there were 5 specimens with high TGF-?3 expressions but P21 negative, and 3 specimens with low TGF-?3 expression but P21 positive, which indicated a negative relative between TGF-?3 and P21 in EMPD(r=0.842,P<0.01). Lymphatic metastasis was more often taking place in TGF-β3 high expression group compared to the low expression group, and 2-year survival rate was lower in the former group, however these conditions reversed for P21 expression.Conclusion TGF-?3 and P21 plays an important role in the mechanism and development of EMPD...
