Study on AIDS Knowledge and Behavior of MSM in Changchun City Background andObjective: The plague of AIDS in China is increasing since the first AIDS case was founded in 1985. New infectious case mainly was found in injection of drugs and sexual transmission. Morbility and mortality is still higher and plague is extended from high risk groups to normal groups. The total AIDS infection case in our country were 183733 untill October 31, 2006, in which AIDS patient were 40667, death were12464. The three transmission pathway were altogether. Reports from January to October in 2006 showed that the main transmission pathway were drugs injection and sexual transmission, the percentage were 37.0% and 28.0% respectively. transmission by blood infusion was 5.1%(all case were infected in 90's and check out in 2006), vertical transmission was 1.4%. The sexual transmission was increasing. It was showed that AIDS infection rate in unlicensed prostitute was 0.02% in 1996, the infection rate in unlicensed prostitute was more than 1.0% in 2005.The survey for MSM showed that HIV infection rate was 1 % - 4%. In some hish risk area the rate of HIV infection in pregnant and marriage people was 1% or even more.The risk factor of AIDS epidemic was still exist. The survey data showed that. the rate of insisting usage of condom in unlicensed prostitute was 38.7%, the rate of joint use of injection unit among the drugs injection was 50.8%. Moreover, the flow of infections, mass population flow and increasing plague of sexual related disease were the important cause of spread of AIDs.Men who have sex with men(MSM) is a term used to classify men who have sex with men, regardless of whether they self-identify as gay, bisexual, or heterosexual. MSM are one of the populations that account for the greatest burden of the HIV epidemic.It was not clear about the MSM's knowledge, attitude, and serology related with AIDS in Changchun city. To understand their knowledge, attitude, behavior and serology related with AIDS is the keypoint for analyzing potential hazard of sex transmission pathway. It will provide scientific reference for estimation of the prevention and making prevention plan and evaluation of prevention fraction of coverage.Methods: The'snowball' survey was used to investigate the MSM's health status. The blood sample was used to test the HIV antibody and syphilis antibody. All the questionnaire data were recorded into EPIDATE3.02 software, and were analyzed by SPSS14.0.Results: There were 408 MSM in this program. They were mainly from bar(44.4%), a bathing pond(25.7%), out-patient clinic of venereal disease(29.9%). The yongest was only 16 yrs, and the oldest was 59 yrs in this test. The average age was 28.6±9.4 yrs, and the median is 25 yrs. The marriage status: unmarried(68.6%), get married(23.8%), cohabitation(3.2%), divorce/ loss of spouse(4.4%). The distribution: Changchun (74.3%), other place in Jilin province (17.4%), other province (8.3%). The nation status: Han people(90.4%), minority(9.6%). The education level: primary school(15.5%), junior school(18.4%), senior school/ technical secondary school (24.5%), college(41.7%). The profession: student(27.9%), teacher(1.2%), catering (7.4%), commerce service (24.3%), medical profession (1.0%), worker(10.5%), migrant workers(5.1%), farmer(3.9%), office workers(8.1%), retirement(1.0%), housework(5.4%), other (4.2%).Sexually, 55.1 % were male-to-male, 20.8% were bisexual, 2.9%failed to clarify their orientation and 7.7% refused to respond. The awareness rate was 23.0% according to UNGASS index, 67.2% according to national standard index, and 67.6% according to AIDS epidemic pathway index.The yougest age at initiation sex among MSM was 13, the oldest was 29, the average age was 20.04±2.68 yrs, the median age was 20 yrs, and semi-quartile range was 4 yrs. 66.9% of MSM were reported that the partner of the initiation sex was male, the yougest age was 14yrs, the oldest age was 26yrs, the average age was 19.43±2.10 yrs, the median age was 19 yrs, and semi-quartile range was 3 yrs. 45.3 % (185/408)of the MSM engaged in anal intercourse with male in previous 6 month, the male partner was from 1 to 30 , and the average was 2.33±3.37, the median was 1 , and semi-quartile range was 1.The usage of condom was 55.1 % when the MSM had anal sex with male last time. 26.9 % of the MSM engaged in protected anal intercourse with male in previous 6 month. And 7.6% of MSM had had sex with female partners in previous 6 month. the female partner was from 1 to 4 , the median was 1. The usage of condom was 56.5 % when the MSM had sex with female partner last time. 21.7 % of the MSM engaged in protected intercourse with female in previous 6 month.45.3% of MSM have had sex with male partners in previous 6 month, 9.2% MSM had got the sex service by money and 6.5% MSM provide the sex service to get money. The usage of condom was 70.6 % when the MSM had"buy sex"with male last time. 25.0 % of the MSM engaged in protected sex with male in previous 6 month. The usage of condom was 75.0 % when the MSM have"sell sex"with male last time. 33.3 % of the MSM engaged in protected sex with male in previous 6 month. 0.2% MSM had drug abuse history, and only one injected drug in previous 6 month.Last year,11.8% of MSM have sexual disease related syndrome. When it happened, 43.8% of MSM bought medicine in the drugstore by themselves. 27.1% of them choose the sexual disease professional hospital. 20.8% of them choose the private clinic. 16.7% of them choose none. 14.6% of them choose polyclinic. In the last year, 37.5% of MSM got free condom, 17.6% of MSM got free lubricant, 20.3% of them got education from partner, 21.3% of them got sexual disease check or treatment, 43.1% of them got free propagandize materials. Most of MSM (56.1%) got condom from drugstore, followed by those who got from adult health care store( 44.9%). The infection rate of syphilis in MSM was 20.1%, and infection rate of HIV was 1.5%. Conclusion1, The yougest age at initiation sex among MSM was 13 yrs. The age was younger than the result from Tao xiaoyan's survey in Shenzhen.2, The usage of condom was 55.1 % when the MSM had anal sex with male last time. 26.9 % of the MSM engaged in protected anal intercourse with male in previous 6 month. The rate was lower than that in 2003 year.3, According to UNGASS index, National standard index and AIDS epidemic pathway index, the awareness rate in MSM of bath/sauna, and MSM of age older than 35, married, townee, minority were rather lower.4, MSM from the sexual disease clinic, age older than 35, married, middle school education, minority showed higher rate of syphilis positive. MSM from bar/night club, 25 years group, unmarried, townee, lower than primary school education, the Han nationality showed higher rate of HIV positive.MSM are very active in sexual. Multi sex partner, unprotected anal intercourse, oral sex and rarely using condom were main HIV/AIDS high risk sexual behavior in those groups. Demology factors variable, have different impact on HIV/AIDS high risk sexual behavior. So we can draw pertinence measure to reduce MSM's high risk sexual behavior bases on these different factors, and then effectively control the prevalence of HIV/AIDS in this groups.
