ObjectiveTo study the change in plasma Brain natriuretide peptide in children withKawasaki disease and its value in clinical practice.MethodsBlood was obtained to measure and compare plasma BNP concentrations in theacute (n=45) and convalescent (n=20) phases of KD, which included classic KD(n=34) and incomplete KD (n=11) and in the acute phase of the patients with viralinfection (control A,n=16)and in the health children (control B,n=10).Otherlaboratory data including WBC,CRP,PLT were measured in both acute andconvalescent phases of KD. In patients with KD, two-dimensional echocardiographywas performed to measure the dimensions of the coronary artery and check forcoronary arterial lesions. Results The mean plasma BNP concentration in patientswith KD in the acute phase was 144.3±82.1ng/L,but was 50.9±25.1ng/L in patients ofcontrol group A, 48.9±23.8 ng/L in patients of control B. The plasma BNPconcentration in patients with KD in the acute phase was significantly higher than thatof the control group A and the control B (P<0.01). In 20 cases of KD, the plasma BNPconcentrations were measured both in the acute and convalescent phasa. The meanplasma BNP concentration in the acute phase of KD was 167.4±154.6ng/L and in theconvalescent phasa was 52.8±44.2ng/L. The level of plasma BNP decreasedsignificantly in the convalescent phase (P<0.01). The mean plasma BNP concentrationin the acute phase of classic KD was 140.8±93.2ng/L and in the incomplete KD was 132.2±26.4ng/L, there was no difference between the plasma of BNP in theclassic KD and the incomplete KD. WBC and CRP in the classic KD was higher thanthat of the incomplete KD. Through linear regression analysis, the BNP levelcorrelated positively with WBC,CRP (r=0.350, r=0.396, P<0.05).ConclusionThe plasma BNP concentration increased in the acute phase of KD and decreasedto within normal range in the convalescent phase; and the BNP level correlated withWBC,CRP. The plasma BNP might be useful of the diognosis and the treatment ofKD, espically incomplete KD.
