Objective:The aim of this retrospective study was to systemically compare the early outcome of between on-pump and off-pump CABG.Design and Methods: Retrospective database and medical record review from January 2004 to June 2006,89 consecutive patients enrolled in the off-pump CABG were compared with 102 consecutive on-pump patients from the first clinical hospital of Xin Jiang Medical University.The two groups were compared the baseline characteristics,operation time, residence time in ICU,intubation time,transfusion of packed RBC,intraoperative and postoperative complications.All values are expressed as mean±standard deviation or percentage.The continuous variables were compared by the t test and Wilcoxon signed rank test,whereas the discrete variables were analyzed with X~2 test and Fisher's exact test to investigate the effect of each technique on clinical outcomes.Results: Preoperative characteristics were similar in both groups,intraoperative differences between the two groups included a higher volume of allogeneic blood transfusion in the on-pump group(4.49±2.56u vs 3.19±2.15u,p=0.034).the operation time in on-pump group was significantly longer than that in the off-pump group(5.04±1.12 h vs 3.69±0.85h,p=0.000).residence time in ICU in off-pump group was shorted than in on-pump group(48.29±28.45h vs 49.15±33.98h,p=0.990).the cost of initial hospitalization for off-pump CABG was significantly less than on-pump CABG (48506.54±1641.37元vs 54808.41±2101.52元,p=0.035).The incidence of perioperative complications in off-pump group significantly lesser than that in on-pump group,the average number of grafts was fewer in on-pump groups than in off-pump group (3.12±0.87vs 2.88±0.75,p=0.031).The time of postoperative recovery in off-pump group was shorter than that in on-pump group(144±32min vs 183±49min,p=0.036)。perioperative overall mortality rate(3.92%vs 3.71%,p=0.890)was comparable in both groups.Hospital length of stay(21.93±7.60d vs 22.34±8.50d,p=0.709)and the time of patients who required mechanical ventilation(13.42±6.89h vs 12.64±6.73h,p=0.307) were similar in two groups.Conclusions:Off-pump coronary artery graft surgery (OPCAB)is as safe as on-pump CABG;The incidence of perioperative complications in off-pump group was significantly lesser than that in on-pump group,and the cost of initial hospitalization for off-pump CABG is less expensive than on-pump CABG.
