Effect Of UHPP On Health Service Utilization To The City Low Living Standard Guarantees

[Purpose] Through the changes of the health service utilization situation before and after the implementation of medicaid to the UHPP medicaid objects, including the outpatient service use and the hospitalized service utilization, to understand the influence factor of the health service utilization, and analysis the problems existed, providing proposals to the next research and the experimental site work.[Method] The article mainly divides into two major parts, the first part, by using the material from the protects and monitor database of our city and society to see whether the health service utilization is equal between the city low living standard guarantees and city low living standard non-guarantees, and to see whether the health service utilization factor of the former one is lower than the latter one. Second part, before the utilization has done in Shenyang, Chengdu's baseline investigation, also has shown that health service utilization of the city low living standard guarantees in Shenyang and Chengdu is lower than that of the city low living standard non-guarantees; then carrying on the medicaid intervention to the medicaid objects, to see the changes of the health service utilization situation before and after the implementation of Medicaid.[Result]The first part, respectively shows the prevalence, the treatment expenditure situation and so on of outpatient service and hospitalized service between the city low living standard guarantees and city low living standard non-guarantees. The result indicates: in the outpatient service aspect, the two groups' the outpatient service expenditure is not equal in the three cities, and so is the hospitalized service. The city low living standard guarantees'is obviously lower than the city low living standard non-guarantees'.The second part, shows the implementation situation of medicaid service's experiment site work in Shenyang. Paying attention to that whether it is different in health service utilization in the poor group with or without the implementation of medicaid .The result indicates: the medicaid intervention causes distinct enhancement of the service use in poor group.[Conclusion]The UHPP project mechanism enhanced the urban poor to the health service utilization and reduced the urban poor's disease economic burden; it has promoted the community health service rapid development, the urban community health service utilization enhancement; It has a good social efficiency.The community health service is still not perfect enough, its Publicity dint degree is weak, and the sustainable development affects the urban poor's health service utilization. We suggest perfecting the community health service's network and the connotation construction; realizing government investment policy to the community health service; enhancing the public welfare of community health service; rescuing the propaganda dynamics thoroughly, perfecting the multi-dimensional fund raising channel; carrying on the dynamic management to the poor group and so on.
