Objective To investigate the serum concentration of IL-18 in patients with type 2 diabetes mellitus and in age-match control subjects,and study wether IL-18 levels are associated with diabetic atherosclerosis ,and a useful serum marker to evaluate carotid artery atherosclerosis.Methods Sixty-six patients with type 2 diabetes and thirty age-and sex-match healthy control subjects were enrolled.IL-18 level were determined by Enzyme-Linked Immuno Sorbent Assay(ELISA). Using the mean +2SD of serum IL-18 in control group as a cutoff value ,patients were divided into IL-18-normal group(n=38),and IL-18-higher group(n=28). Using high sensitive B-mode ultrasonography detecting of the IMT of the patients with type 2 diabetes , patients were divided into carotid artery -think group(n=27),and carotid artery-normal group(n=39).Using homeostasis model assessment(HOMA-IR) estimated insulin resitance. We assessed clinical parameters and measured serum IL-18 level ,carotidintima-media thickness(IMT)in type 2 diabetes patients.Result Serum IL-18 was singnificantly higher in diabetic patients than in control subjects IL-18{ 90.31 vs 71.34pg/ml,M(QI) ,P<0.05} .Patients with high IL-18 showed a greater carotid IMT(0.92±0.22 vs0.77±0.19 mm ,P<0.01)and FBG(9.62±2.4 vs. 7.93±2.5 mmol/l ,P<0.01) than with normal IL-18. Serum IL-18 was singnificantly higher in diabetic patients with carotid artery -think group than with carotid artery -normal group. Univariate linear regression analysis showed significant positive correlations between serum IL-18 and IMT(r=0.398,P=0.001), FBG(r=0.465,P<0.001) and HOMA-IR(r=0.399, P=0.001).Univariate linear regression analysis showed significant positive correlations between IMT and FBG(r=0.343,P=0.005),TC(r=0.329,P=0.007), LDL-C(r=0.360,P=0.003) significant negatively correltions between IMT and HDL-C(r=-0.334 ,P=0.006).Conclusion Serum IL-18 is linked independently with atherosclerotic carotid wall thickening and plays pivotal roles in pathogenesis and development of atherosclerosis and type 2 dibetes. Serum IL-18 may be a useful serum marker to evaluate carotid artery atherosclerosis.
