A Study Of The Relationship Among Cervicovaginal Interlukin-8, Matrix Metalloproteinase-8 And Preterm Labor

Otbjective:investigate the realationship among Cervicovaginal Interlukin-8, Matrix Metalloproteinase-8 and Preterm labor .Method: Samples of cervical secretion were obtained from 45 patients with threatened preterm labor (study group) and 42 patients with normal pregnancy(control group), the gestational weeks of both groups are similar. IL-8and MMP-8 were measured by ELISA.Results: (1) IL-8 and MMP-8 levels of the study group were higher than those of the control group, P<0.05;IL-8 and MMP-8 levels of the group with preterm labor were higher than those of the group with term labor, P<0.05;(2) There was a significantly positive correlation between the cervicovaginal concentrations of IL-8 and MMP-8, r=0.671 , P<0.05 ; (3) IL-8 had a sensitivity of 88.5%, a specificity of 84.3%, a positive predictive value of 73.7%, and a negative predictive value of 91.5% in predicting preterm delivery and had a sensitivity of 82.2%, a specificity of 80.5%, a positive predictive value of 60.7%, and a negative predictive value of 94.5% in predicting delivery within one week. MMP-8 had a sensitivity of 76.9%, a specificity of 66.7%, a positive predictive value of 50%, and a negative predictive value of 83.8% in predicting preterm delivery and had a sensitivity of 75%, a specificity of 60.4%, a positive predictive value of 52.8%, and a negative predictive value of 90.4% in predicting delivery within one week. (4) For both IL-8 and MMP-8,Compared with the negative group ,the positive group had the much less delivery gestational weeks;the lower neonat weights;and the shorter delivery intervals.Conclutions: (1) Both cervicovaginal level of IL-8and MMP-8 were correlated with preterm delivery. (2) There are significantly positive correlation between the cervicovaginal level of IL-8 and that of MMP-8 (3)both IL-8 and MMP-8 were effective markers for prediction of preterm labor and delivery within one week ,but the predictive ability of MMP-8 was relatively weak.
