Objective To analyze the relationship between COMP and the degree of articular cartilage injury and synovitis in a cross-sectional study, and research the role of COMP in osteoarthritis(OA).Method 45 patients with knee osteoarthritis(OA group),49 other patients with knee other disease-arthroscopically verified injury to meniscus,ACL,PCL and so(non-OA group)and 67 healthy volunteers(normal group)were studied. The level of COMP was measured by enzyme linded immunoassay. The degree of cartilage injury was assessed with Outerbridge score system and the degree of synovitis with Ayral score system under arthroscopy. The data was analyzed by t-test or nonparametric test, analysis of variance,Pearson or Spearman correlation. Results The levels of COMP in the serum of three groups were studied usig analysis of variance, then the result is F=20.982;P<0.05.By SNK statistical analysis, there is difference in OA group and control groups, that is,the content of COMP in non-OA group is highest;there is not obvious difference in normal group and non-OA group. The content in serum in OA correlated positively with cartilage injury scores(r=0.66,p=0.000<0.05)and synovitis scores (r=0.57,p=0.000<0.05),and in 103 patients, the content of COMP correlated positively with synovitis scores(r=0.60,p=0.000<0.05).Furthermore,the content of COMP correlated positively with the age of patients(r=0.57,p=0.000<0.05)in OA group. Conclusion The content of COMP which is a cartilage-specific protein in serum can reflect the degree of accumulative cartilage injury as a biomarker. In the essay, the content of COMP in serum in OA was higher than the control groups'.Probably to suggest the content of COMP in serum increased in the early of OA.The risk factor of Cartilage injury in early result in OA in the final. But the mechanism of increased level of COMP in serum remains unclear.
