The Diagnostic Value Of 12-lead AECG In The Coronary Heart Disease (CHD), And Clinical Application Of 12-lead AECG After PCI

Objective:To evaluate the diagnostic value of 12-lead AECG in the coronary heart disease(CHD),and clinical application of 12-lead AECG after PTCA and Intracoronary stent implantation.12-lead AECG was monitored to observe the change of total ischemic burden(TIB)before PCI and different stages after PCI.we aim to research the influence of PCI on myocardial ischemic of CHD and to seek to find a harmlessful way to evaluate the clinical effect of PCI.Methods:Fifty patients were examined by CAG and 12-lead AECG.The duration of the both methods was less than one week.Base on coronary angiography(CAG),ST-segment depression data of 12-lead AECG were compared with the findings demonstrated by CAG, the sensitivity and specificity for detection of CAD with 12-lead AECG were computed. Additionally,12-lead AECG monitoring was done in another 30 patients who accepted successful PCI before PCI and at one week,one month,three months after PCI respectively,we aim to analyze the change of myocardial ischemic before PCI and different stages after PCI.Results:1.with the CAG as the standard,the sensitivity and specificity of 12-lead AECG in the diagnosis of CHD were 77.7%,85.7%respectively,the sensitively of 12-lead AECG on CHD correlated Closely with extent of CHD,the incidence of myocardial ischemia in patients with multi-vessel disease is higher than with one-vessel disease.2.the frequency,the duration,the maximal ST-segment depression of the myocardial ischemic and total ischemia burden(TIB)in multi-vessel group compared with that of single vessel group were significantly different between the two groups(p＜0.05).3.TIB was not significantly improve at one week after PCI,but the frequency,the duration,the maximal ST-segment depression of the myocardial ischemic and TIB at one month,three months after PCI compared with that before PCI were significantly decreased.4,the incidence of both silent myocardial ischemia and angina myocardial ischemia at different stages after PCI compared with that before PCI were significantly decreased,the incidence of the silent myocardial ischemia were significantly higher than that of the angina myocardial ischemia at different stages after PCI.Conclusions:1.the sensitivity,specificity of 12- lead AECG in the diagnosis of CAD were 77.7%,85.7%respectively,so it is an effective non-invasive method in the diagnosis of CAD.2.PCI can significantly reduce the frequency and extent of myocardial ischemia on patients of CAD.
