Expression Of Matrix Metalloproteinase-2 And Tissue Inhibitor Of Metalloproteinase-2 In The Placenta Of Hypertensive Disorder Complicating Patients

ObjectiveThrough studying the expression of MMP-2 and TIMP-2 in theplacenta of the women with normal pregnancy and hypertensive disordercomplicating pregnancy patients, we probe into the relationship ofincidence mechanism of hypertensive disorder complicatingpregnancy.It will make us more understand pathogeny of hypertensivedisorder complicating pregnancy and will inaugurate a new way for thepreventing and diagnosis-therapy of hypertensive disorder complicatingpregnancy.MethodThe placenta tissues are from the women with delivery in theNanhua Affiliated Hospital of Nanhua University and genecology andobstetics hospital of hengyan from Oct. 2005 to Apr. 2006. The totalnumber is 52, and 16 of it is from the women with normal pregnancy(control team) and 36 of it is from the women with hypertensive disordercomplicating (experimentation team).The number of mild pre-eclampsia.severe preeclampsia and eclampsia are respective 15,15,6.they all have not the ill history of primary hypertension,diabetes,chronic nephritis and so on The diagnostic standard ofhypertensive disorder complicating refers to the NO.6 edition《genecology and obstetics》compiled by Lejie. After the delivery of pregnant women, we collect placenta tissues immediately and fix themby 10% formalin and embed them by paraffin。We perform animmunohistochemical method to detect the positive expression ratio ofMMP-2 and TIMP-2 in the placenta of the women with normalpregnancy and hypertensive disorder complicating pregnancy, andanalyse the data by statistical software.Results1. The positive expression of MMP-2 and TIMP-2 is in the plasmaof syneytiotrophoblast cell, trophoblast cell,mesenchyme cell, vascularendothelial cell of the placentas in normal pregnancy woman group andhypertensive disorder complicating group. But the expression of MMP-2is mostly weak positive in the hypertensive disorder complicating group.The expression of TIMP-2 is still positive in the hypertensive disordercomplicating group. The positive expression ratio of MMP-2 andTIMP-2 in the trophoblast cell is higher in the placentas of women withboth the normal pregnancy group and hypertensive disordercomplicating group.2. Immunohistochemical scores (IH score) of MMP-2 introphoblast cells among four groups:normal pregnancy group(5.81±0.40),mild preeclamptic group (4.47±0.52),severe preeclampticgroup (1.87±0.52),eclamptic group (1.67±0.52).There were differencebetween the normal pregnancy group and hypertensive disorder complicating group (F=220.46, P<0.001). There were difference amongthe mild preeclamptic group and the severe preeclamptic group,theeclamptic group (P<0.001), There were not difference between thesevere preeclamptic group and the eclamptic group (P>0.05)。3. Immunohistochemical scores (IH score) of TIMP-2 introphoblast cells among four groups:normal pregnancy group(5.63±0.50), mild preeclamptic group (5.40±0.51), severe preeclampticgroup (5.27±0.70),eclamptic group (5.17±0.75).There were notdifference among four groups (F=1.31, P>0.05)4. The average sum of ranks of ratio of TIMP-2 and MMP-2among four groups: The average sum of ranks in the normal pregnancygroup is 9.88, The average sum of ranks in the mild preeclamptic groupis 22.53, The average sum of ranks in the severe preeclamptic group is41.47, The average sum of ranks in the eclamptic group is 43.33. Therewere difference between the normal pregnancy group and thehypertensive disorder complicating group (H=43.47, P<0.001). Therewere difference among the mild preeclamptic group and the severepreeclamptic group,the eclamptic group (P<0.001), There were notdifference between the severe preeclamptic group and the eclampticgroup (P>0.05).The positive expression ratio of TIMP-2 and MMP-2 inhypertensive disorder complicating pregnancy is significantly higherthan normal pregnancy team, and with the aggravation of state of an illness, the positve expression ratio in hypertensive disordercomplicating pregnancy is increased gradually.Conclusion1. The positve expression of MMP-2 in hypertensive disordercomplicating pregnancy is reduced than normal pregnancy woman, thedegree of the positive expression of MMP-2 may be correlated with stateof hypertensive disorder complicating pregnancy.2. The positive expression of TIMP-2 has not significant differencein hypertensive disorder complicating with normal pregnancy woman.3. The heighten and imbalanced proportion of TIMP-2 andMMP-2 may be one of the reasons that lead to the obstruction oftrophoblastic recoinage in hypertensive disorder complicatingpregnancy.
