Investigation And Analysis Of Health Resource And Efficiency Of Using Health Resource In Town Hospitals In Two Counties Of Hunan Province

Objective: In order to providing the reasonable political suggestions for improving theinvestment condition to health resources of town hospitals and enhancing the efficiency to it inHunan province, we investigated two kinds of town hospitals (central hospital and ordinal hospital)in the two representative counties (Ningxiang county and Wangcheng county) of Hunan province,and comprehend their health resources and present conditions.Methods: Through using the method of questionnaire and letter, all the central hospital andordinal hospitals in the tow counties were investigated, that is 54, of which there are 7 centralhospitals, 28 ordinal hospital in Ningxiang County and 5, 14 corresponding in the other. Thecontent of investigation includes general condition, all peoples' formal schooling and professionaljob title, the possession and utilizing of medical instruments, and so on.Results: 54 questionnaires were delivered and returned all together, of which 7 weredelivered in central hospitals, 28 in ordinal hospitals of Ningxiang County and 5, 14corresponding in the other. The technical personnel of ordinal hospital in the two counties is 825,of which 526 in Ningxiang County, 299 in Wangcheng County. Among their record of formalschooling, the majority is technical secondary school in Municipality and County level,accounting for 52.2% (38.8% in Wangcheng County and 59.9% in Ningxiang County), and overhigher education only accounts for14.1%(11.7% in Wangcheng county and 15.4 in Ningxiangcounty),but no medical subject accounts for 33.7%(49.5%in Wangcheng county and 24.7% inNingxiang county).Chi-square test show that the formal schooling composition of ordinalhospitals in the two counties exists distinct differences,that is ,Ningxiang county is higher thanWangcheng county .the technical personnel of central hospitals in the two counties is 562,of which 392 in Ningxiang county and 170 in Wangcheng county .Among their record of formalschooling ,the majority is technical secondary school in municipurity ,accounting for58.0%(54.1% in Wangcheng county and 59.7% in Ningxiang county),then the higher educationaccounts for 24.9%(25.3% in Wamgcheng county and 24.8% in Ningxiang county),the two kindrecord of formal schooling is 82.9% in all. Chi-square test indicates that there is no remarkabledifferences in the record of formal schooling of the central hospitals in the two counties.The maincomposition of technology and professional job title is elementary, over accounting for 42.5%,nextis middle level ,accounting for 25.2%,senior and vice-senior account for 1.2%.No professional jobtitle accounts for 18.3%.Chi-square test indicates that there exists remarkable differences in thecomposition of technology and professional job title of ordinal hospitals in the two counties, andNingxiang county is superior to Wangcheng county; and the main composition of technology andprofessional job title in central hospitals is junior, accounting for 46.8%, next is middle level,accounting for 37.0%, senior and vice-senior all accounts for 2.0%, the final investigationindicates that there also exists distinct differences in the composition of technology andprofessional job title of ordinal hospitals, and Wangcheng County is superior to NingxiangCounty.As for as the medical instruments of ordinal hospitals in the two counties, X-ray machine isuniversal to use, the average is 2.2 in Ningxiang County, 1.4 in Wangcheng County; dry-trunk isalso popular to use, the average using rate in the two counties is 78.6% (64.3% in Ningxiangcounty and 100% in Wangcheng county);the possession rate of electrocardiogram is lower ,theaverage possession rate in the two counties is 14.3%(21.4% in Ningxiang county and 0% inWangcheng county);the lowest possession rate machine is multifunction parturition-bed andnewborn emergency-desk, only 4.7%.In general ,the owning-scale to the medical equipments inthe ordinal hospitals is less,and Chi-square test showed that ordinal hospitals in Wangchengcounty were equipped with more medical equipments than that in Ningxiang county .The centralhospitals medical equipments in the two counties are initially forming the scale ,and are much superior to ordinal hospitals (of which specific situation is seen in table 11),and Chi-square testshowed that central hospitals in Ningxiang county were equipped with more medical equipmentsthan that in Wangcheng county.With respect to the distribution of medical equipments,their all circumstances are severe. Forexample, at all the 58 hospitals, only two ordinal hospitals in Ningxiang county possess one each,the rest are all no corresponding match, that is only 3.70% possessing it;in addition ,all kinds ofequipment distribution that operations need have higher owning-rate ,which to certain extentaffect the improvement of all hospitals' surgeon . In the other hand, with regard to healthyresources' utilization ratio, all of the town hospitals are basically low-resources and low-utilizationratio, such as ,the highest utilization ratio to hospital beds is central hospitals in Wangchengcounty, accounting for 64.1%,but the lowest is central hospitals in Ningxiang county, whoseaverage opening hospital beds are 47.6 to the highest extent, and the ordinal hospitals in that arein least 11. Furthermore, hospital beds of all the hospitals in Ningxiang county and of ordinalhospitals in Wangcheng county are set aside over half.Conclusions:Educational level of medical staff in the two counties is relatively low ingeneral ,and their technology is not too high . Healthy resources in the two counties is alsorelatively inadequate, and their distribution is unbalance. In general ,central hospitals are equippedwith more healthy resources than ordinal hospitals ,and that of Ningxiang county is higher thanWangcheng county. In respect to hospitals'resources utilization, all hospitals are low-efficiencybecause of shortage of resource deployment, relatively low-technology of medical staff and lackof efficient management.
