Objective: We studied the effect of different oxygen concentrationon the activity of serum superoxide dismustase (SOD),the concentrationof serum malonydialdehyde (MDA) and artery oxygenation ofepigastric operation patients and clinical function of propofol, inorder to select the proper oxygen concentration and vein sedatives.Method: Eighty selected gastric cancer patients, receiving radicaloperation, ASAⅡ, were randomly divided into eight groups(n=10).According to oxygen concentration, patients were divided into35%FiO2(A),50%FiO2(B),75% FiO2(C)and 100% FiO2(D). Every oxygenconcentration was divided into two groups.Group A1, B1, C1 and D1 weregroup P that used propofol during operation. Group A2, B2, C2 and D2were group UP that were sustained by midazolam. There were nosignificant differences in general conditions in all the groups. Differentinhaled oxygen concentration were generated by breathing machinefrom induction. Blood samples were colleted from peripheral vein atdifferent time points including 5 minutes before induction(T0), 60minutes aider induction(T1),120 minutes aider induction(T2) and the endof the operation(T3). At the same time artery blood samples were drawnto test PaO2 and PaCO2. The venus blood samples were collected whichwere stored by refrigerator under-70℃.The serums were used for measuring MDA level and SOD activities. Venus serums of tennone-tumor patients were collected as control(group C).Result:1. There were no significant differences in general conditions in allthe groups(P>0.05).2. The changes of SOD and MDA: There were no significantdifferences of SOD and MDA before induction(P>0.05). 1) The activitiesof SOD in groups A1,B1,C1 and D1 were decreasing at first. After 2 hoursthey increased gradually but lower than the level of 60 minutes afterinduction. The contents of MDA were increasing at first. After 2 hoursthey decreased but higher than the content of 60 minutes after induction.2) The activities of SOD were decreasing and the contents of MDA wereincreasing all the time with the inhaling time increasing in group A2, B2,C2 and D2.3) Compared with the level before induction, the decreasingof the activity of SOD had significant deviation at the end of operation ingroup A2, at 120 minutes after induction in group B2, at 60minutes ingroup C2 and DE(P<0.01). Compared with group A2, the serum SODactivity decreased at the end of operation in group B2 and at 120 minutesin group C2 and D2(P<0.01). Compared with the level before induction,the decreasing of the activity of SOD had significant deviation at 120minutes after induction in group B1, at 60minutes after induction ingroup C1 and D1(P<0.01). Compared with group A1, the serum SOD activity decreased at 60 minutes after induction in group D1(P<0.05). 4)Compared with the content before operation, the increasing of the contentof MDA had deviation at the end of operation in group A2, at 120 minutesafter induction in group B2 and C2, at 60 minutes after induction in groupD2(P<0.01).Compared with group A2, the content of MDA increased atthe end of operation in group B2 and at 120 minutes in group C2 and at 60minutes in group D2(P<0.01). Compared with the content beforeoperation, the increasing of the content of MDA had deviation at end ofoperation in group A1, at 120 minutes after induction in group B1 and C1,at 60 minutes after induction in group D1(P<0.01). Compared with groupA1, the concentration of MDA increased at 120 minutes after induction ingroup D1(P<0.05).4. PaO2 in all groups increased accordingly as the inhaled oxygenconcentration increased. PaO2 of group A1 and A2 increased all the time,but other groups at the end of operation were lower than 60 minutes and120 minutes after induction, higher than preoperative. There were nosignificant differences in group P and UP.5. Contrast to the control group, the activities of SOD of stomachcancer patients were lower and the concentration of MDA werehigher(P<0.01).Conclusion:1. With time and concentration of inhaling oxygen increasing, free oxygen racidal increased while the activity of SOD decreased on thecontrary. The oxygen concentration of 35% has the smallest effect on theSOD and MDA of epigastic operation patients.2. Propofol has antioxidation activity, it can suppress lipidperoxidation and enhance the activity of SOD. Though it can'tretrieve the imbalance completely, it inhibits the generation of the oxygenfree racial of some extent. Propofol is an excellent vein narcotic.3. With concentration increasing, PaO2 had increased. It wasincreasing continuously in concentration of 35% during operation. Itmay has relation with high oxygen concentration which made the freeradical increasing and obstructive atelectasis aggravating.4. There was imbalance of peroxidation-antioxidation in tumorpatients.
