Objective: To analyze the correlation between IL-1βand degree ofOsteoarthritis, substance P and degree of Osteoarthritis, IL-1βandsubstance P. To approach the correlation and possible involvement ofIL-1βand substance P in pathogenesis and progression of humanosteoarthritis (OA). To observe the affection of exogenous hyaluronicacid on IL-1βand substance P in knee synovial fluid of patients. Toexplore a possible mechanism of treating osteoarthritis with exogenoushyaluronic acid.Methods: To draw knee synovial fluid of 50 OA patients randomlyfor group B, according to synthesis score, mild group 15 eases, moderategroup 20 cases and severe group 15 cases. To draw knee synovial fluid of6 acute trauma patients randomly for control group. 30 OA patients candraw knee synovial fluid after treatment with hyaluronic acid from 55 OApatients for group C. According to synthesis score, mild group 7 cases,moderate group 15 cases and severe group 8 cases. Contents of IL-1β,substance P before and after OA treatment were measured by ELISA, andcompared with those in the controls.Results:1. Joint fluid contents of substance P in group A and group B wererespectively68.70±11.96pg/ml, 99.44±25.35pg/ml. Joint fluid contents of substance P in group B were elevated with a significant difference(p<0.05).2. Joint fluid contents of IL-1βin group A and group B wererespectively 25.89±5.40pg/ml, 39.09±14.72pg/ml. Joint fluid contents ofIL-1βin group B were elevated with a significant difference (p<0.05).3. There were direct correlation between IL-1βand degree ofOsteoarthritis(r=0.79, p<0.01), between substance P and degree ofOsteoarthritis(r=0.76, p<0.01), between IL-1βand substance P(r=0.83,p<0.01).4. Joint fluid contents of IL-1βand substance P showed nosignificant difference(p>0.05) between mild and moderate beforetreatment. Joint fluid contents of IL-1βand substance P showedsignificant difference(p<0.01) between moderate and severe group beforetreatment.5. Joint fluid contents of IL-1βand substance P showed significantdifference(p<0.01) between pre-operative and post-operative. Joint fluidcontents of IL-1βin mild group showed no significant difference(p>0.05)between pre-operative and post-operative. Joint fluid contents ofsubstance P in mild group showed significant difference(p<0.05) betweenpre-operative and post-operative. Joint fluid contents ofIL-1βin moderategroup showed significant difference(p<0.01) between pre-operative andpost-operative. Joint fluid contents of substance P in moderate group showed significant difference(p<0.05) between pre-operative andpost-operative. Joint fluid contents of IL-1βin severe group showedsignificant difference(p<0.05) between pre-operative and post-operative.Joint fluid contents of substance P in severe group showed significantdifference(p<0.01) between pre-operative and post-operative.Conclusions: IL-1βand substance P play an important role inosteoarthritis, There were direct correlation between IL-1βandosteoarthritis, between substance P and osteoarthritis, between IL-1βandsubstance P in knee synovial fluid of osteoarthritis patients. The level ofIL-1βand substance P in knee synovial fluid of osteoarthritis patientsafter treatment with hyaluronic acid was lower than their level beforetreatment. Down-regulation the level of IL-1βand substance P in kneesynovial fluid was a possible mechanism of treating osteoarthritis withexogenous hyaluronic acid.
