Objectives: To study the relationship between the Chinese medicine syndromes of MS and IR,FFA, and exploring the difference of syndrome and type in such patients, which provide the dialectical way and the theoretical basis of combined medicine treatment.Methods: We selected 80 cases, and divided them into five Chinese medicine syndrome groups which include liver-fire exuberant syndrome, phlegm -dampness abundant syndrome, Qi and Yin deficiency syndrome, deficiency of Yin and Yang syndrome, phlegm accumulation and blood stasis syndrome, 20 healthy people were taken as a normal control group. we take their blood pressure,height,weight and waistline. We detected fasting plasma glucose (FPG), triglyceride (TG), high density Hpoprotein-cholesterol (HDL-C), fasting Insulin (FINS), free fatty acid (FFA) through taking suction from Venous blood. The insulin sensitivity index values: ISI=-ln/(FPG×FINS). Early the next morning collect 3 ml urine specimens and add it into dry vitro to detect MAU.Results: The FFA in MS group is higher than the control group (p <0.05), the heightening level of FFA was deficiency of Yin and Yang syndrome> Qi and Yin deficiency syndrome > phlegm accumulation and blood stasis syndrome >phlegm-dampness abundant syndrome >liver-fire exuberant syndrome (p<0.01), the ISI of MS group was significantly lower (p<0.05),the degree of ISI lowing level was Yin and Yang syndrome> Qi and Yin deficiency syndrome > phlegm accumulation and blood stasis syndrome >phlegm -dampness abundant syndrome> liver-fire exuberant syndrome (p<0.01).Conclusions:1. The high FFA is connected with the IR in MS groups, it probably caused the MS group other pathophysiological changes.2. FFA levels can be used as MS - TCM Syndrome laboratory based.3. Phlegm and stagnant blood are the mainly important pathogenic factor of MS.
