Effect Of Gufuhuotang On ET-1, NO In Steroid-induced Avascular Necrosis Of The Femoral Head

Objective:To observe ET-1,NO in the steroid-induced avascular necrosis of the femoral head (SANFH) and the effect of Gufuhuotang on vascular endothelial in SANFH.Methods: The animals, randomly divided into four groups, 8 each: Control group(CG), Model group(MG), Gufuhuotang group ( 15g. kg-1 )(GG) and Xianlinggubao group (0.6g.kg-1)(XG), were given intragluteal injection with prednisone acetate of 7.5mg/kg twice a week, and CG were injected with normal saline of the same volume twice a week, while GG as well as XG were administered with Gufuhuotang and Xianlinggubao very day. In case of infection, all the animals were injected with penicillin (5*104unit/ each) twice a week for 6 weeks. They were weighed once a week and the fur condition and activities were observed. Blood samples were collected from heart puncture immediately after experiment completely.ET-1, NO were measured. The histopathological changes of femoral headof all rabbits were observed after the end pointof experiment. Statistics: All data of four groups, by SAS statistical soft ware, were given a test of normality, homogeneity test of variance, analysis of variance, Q test, and the data were expressed with mean standard deviation ((x|—)±S).Results: Normal morphologic observation: during the experiment, the weight of CG increased gradually;MG decreased, subcutaneous fat reduced;GG and XG decreased a little, and the state was better than MG .6 weeks later, compared to model group, there was significant difference between the weight comparison (p<0.05), but there was no obvious difference between CG, GG and XG (p>0.05). The variety of the blood plasma ET-1 and NO content: Compared to control group, the ET-1 level increased while the NO level decreased. about ET-1, analysis of variance: F=6.49, P=0.0018, there was significant difference between; Q test results: compared to CG, GG and XG, the ET-1 level of MG was significantly different (p<0.05); there was no significant difference between GG and XG (p>0.05); compared to CG, both GG and XG had significant difference (p<0.05). about NO, analysis of variance: F=7.33, P=0.0009, there was significant difference between; Q test results: compared to CG, GG and XG, the NO level of MG was significantly different (p<0.05); there was no significant difference between GG and XG (p>0.05); compared to CG, both GG and XG had significant difference (p<0.05).About empty lacuna: of CG, nucleus of osteocytes evenly pigmentized, osteocytes in lacuna were normal, intramedullary osteocytes moderately distributed; of MG, nucleus of osteocytes had no pigmentation, and there were plenty of empty lacuna; of GG, osteocytes degenerated to be flat deviated to one side, nucleus partly pigmentized, osteocytes distributed less than CG, and there were less empty lacuna; of XG, osteocytes degenerated to be flat deviated to one side, nucleus pigmentized in few parts, osteocytes distributed less than GG, and there were less empty lacuna. The rate of empty lacuna of CG was 12.01±2.13, MG 26.03±4.54, GG 16.50±2.56, XG 17.51±2.98; analysis of variance: F=26.95, P=0.0001, there was significant difference between; Q test results: compared to CG, GG and XG, empty lacuna rate of MG was significantly different (p<0.05); there was no significant difference between GG and XG (p>0.05); compared to CG, both GG and XG had significant difference (p<0.05).Conclusion: The increase of ET-1 and the decrease of NO might play an important role in the pathogenesis of SANFH,disorder defect of vessel endothelial cell were the cause of thrombosis in femoral head ourish vessel,which can be prevented by Gufuhuotang.
