ObjectiveTo explore the value of ultrasound and ultrasound-guided biopsy in diagnosing T1 breast cancer.MethodsCollected 89 cases small breast tumors(malignant 36 cases,benign 53 cases),the sizes of which were within 2cm in diameter.①Use ATL-5000 ultrasound diagnostic instrument,apply SonoCT and Color Doppler flowing imagine detecting the 89 breast lesions.To observe the size, amount,echo,boundary,aspect ratio(H/W)and microcalcification of the breast lensions; evaluate hemodynamic parameters of tumorous vessels:Peak systolic velocity(PSV)and Resistance index(RI)determined by the images of those breast tumors of ultrasound.Finally,all the above mentioned parameter were controlled by pathological diagnosis.②The breast tumors of 20 patients were punctured by automatic biopsy gun guided by color Doppler ultrasound and analyzed with pathological examination.ResultsCombined two dimensional imagine(used SonoCT)and Color Doppler flow imaging (CDFI),the sensitivity,specificity and diagnostic accuracy of the 36 breast tumors is 92%(33/36),94%(50/53),93%(83/89).The appearance of speckles which was significantly reduced in SonoCT resulted in improvement of contrast resolution.SonoCT was also found to suppress artifacts and to reinforce real structures in the breast solid masses,so that the lateral boundaries of solid masses were particularly improved by SonoCT.Smaller malignant lesions in the breast would be detected with SonoCT.Compare to benign mass,T1 breast cancer has some characters such as coarse boundary,microcalcification in mass,H/W>1.They have remarkable significances(P<0.05).In blood flow,it has significant difference between malignant and benign masses (P<0.01).In benign group,most were Grade O blood flow,in malignant group,most were GradeⅡblood flow.Use pulsed wave(PW)detected,benign mass PSV 11.86±2.89cm/s,RI0.59±0.10; malignant mass PSV16.30±6.33cm/s,RI0.76±0.12.There were the significant differences in PSV and RI(P<0.05).PSV≥12crn/s,RI≥0.7 and irruptive blood flow are characteristics of malignant masses(P<0.05).The mass tissues of 95%(19/20)patients were acquired firstly and diagnosed by pathology. Compared with the results of postoperative pathology,the accuracy rate of preoperative pathological diagnosis was 100%.ConclusionsSonoCT can enhance image quality by reducing artifacts.Compared to convent ional ultrasound,SonoCT has the potential positive detection rate for the diagnosis of breast masses.Combining 2D ultrasonography and CDFI,if rich and high arterial blood flow velocity and elevated RI is regarded as a criterion for the T1 breast cancer.The automatic biospy guided by color ultrasound in diagnosis of breast masses has many advantages,such as simple technique,accurate diagnosis and few complication.Comprehensive evaluation can increase the overall accuracy in diagnosing the T1 breast cancer sonographically from indirect sign and by hemodynamics changes of the mass.
