A Nested Case-Control Study On The Influence Factors Of Perinatal Transmission Of HBV Among HBsAg-positive Pregnant Women

Objective: To study the influence factors of perinatal stage transmission of HBV in HBsAg-positive pregnant women, and to provide scientific evidence for perinatal stage transmission of HBV prevention. Methods: A 1:1 matched nested case-control study was conducted. 141 pair of pregnant women with HBsAg-positive and their newborns were selected. From Oct 1, 2005 to Sept 31, 2006, A questionnaire survey was conducted, collecting and testing blood-related indicators. The data was analyzed using Paired Chi-square test and conditional logistic regression analysis via SPSS 13.0.Results: Neonatal Perinatal infection rate was 24.3%. Single factor Paired Chi-square test showed education, first class family history, liver disfunction, serum glutamic - pyruvic transaminase, systematic treatment, ICP, PROM, fetal distress and Vaccination HBIG ,et al were the risk factors of perinatal stage transmission of HBV. The value of (?)R and 95%CI of OR was correspondingly 0.529 (0.332~0.844),2.900 (1.745~4.821),1.947 (1.120~3.386),2.250 (1.140~4.441),0.405 (0.230~0.711),2.333 (1.274~4.272),2.250 (1.249~4.054),1.920 (1.184~3.113) and 0.385(0.230~0.644)The result of conditional logistic regression analysis indicated first class family history, Vaccination HBIG,systematic treatment,PROM and ICP were the risk factors of perinatal stage transmission of HBV, The value of (?)R and 95%CI of OR was correspondingly 2.870 (1.537~5.357),0.294(0.150~0.576),0.374(0.180~0.775),2.278(1.088~4.771)及2.821(1.213~6.561)。Conclusions: For the women with HBsAg-positive, Active treatment,the standard vaccination HBIG and preventing and controlling the incidence of lCP may reduce the incidence ofperinatal stage transmission of HBV...
