Abstract/Summary:
Objectives:1. To research spleen-stomach deficiency, its symptoms and distributioncharacteristics on children leukemia;to increase knowledge about spleendeficiency, clinical signs and symptoms and their roles:to increase knowledge on spleen deficiency analysis and diagnosis and to progress spleen deficiency diagnosis and treatment following scientific principles.2. To investigate the changes of sugar chain of urinary amylase in children leukemia with spleen-stomach deficiency,and its relation to FreeRadical, to find out how the sugar chains reflect spleen-stomach deficiency.Method:Children leukemia receiving chemotherapy, Firstly according to signsand symptoms,to design the four examination registry table for each patient, collecting four examination method data for Traditional Chinese Medicine diagnosis. Divide the cases that meet the criterion in two spleen-stomach deficiency and hepatic qi stasis, Lectins-binding ratios of urine amylase,urinary amylase activity,ONO0-,MDA were determined in children leukemia with spleen-deficiency and hepatic qi stasis, children non-leukemiapatients.Using statistical methods to analyse the finding from spleen-stomachdeficiency patients for examination and laboratory data:to explain spleen-stomach deficiency, its symptoms and distribution characteristics on leuk emia; to explain the sugar chains reflect spleen deficiency and its relation to Free Radical.All the quantitative data are represented by means and standarddeviations. The comparative mean between two groups used the independentsample t test(t' test).The comparison between the mean of three groups used one factor ANOVA analysis. Between groups 2*2 comparison analysis SNK, LSD method was used. Qualitative data used frequency and frequency values.Chi-square(x±s). Two variable values used Pearson correlation to analyse quantitative test range,α=0.05.Results:1. The means of urinary amylase activity in leukemia group (353.44±407.04) spleen-stomach deficiency group (324.90±407.66),and the patient in hepatic qi stasis group (392.18±418.23),both were higher than the controlgroup (141.84±94.42).but the difference is quiet. (p>0.05).2. The patient in leukemia group, PNA,PSA,ConA-binding ratios of urine amylase(0.45±0.15,0.55±0.19,0.46±0.18) and spleen-stomach deficiency group, LCA,PNA,PSA,ConA-binding ratios (0.6±0.16,0.51±0.12,0.58±0.17,0.5±0.17) were significantly higher than the control group (0.48±0.21,0.33±0.21,0.4±0.17,0.34±0.16). the P value of leukemia group were0.01, 0.018,0.041 and the spleen-stomach group were 0.045,0.026,0.031,0.048,all were less than 0.05. no significant variation in the hepatic qi stasisgroup.3. The urinary ONOO~- in both leukemia group (13.15±25.75) and hepatic qi stasis group(11.37±33.83) were lower than control group(34.13±40.15), Pvalue were 0.032 and 0.049<0.05. and the urinary MDA of leukemia (1.22±1.77) and spleen-stomach deficiency group (1.04±1.48) were both lower than the control group(10.8±18.64),P value were 0.019 and 0.04<0.05.4. The patient in both leukemia group and spleen-stomach deficiency group, urinary amylase activity was significantly positive correlation with MDA, the coefficient of correlation was 0.383 and 0.657, p<0.05, but thecorrelation was not manifest with ONOO~-. hepatic qi stasis group, controlgroup both were not manifest with MDA, ONOO~-, p<0.05. A positive correlation was found between PNA, PSA, ConA-binding ratios and ONOO~-.a negative correlation was found between WGA-binding ratios and MDA.5. To compare the symptoms feature between spleen-stomach deficiency group and hepatic qi stasis group in leukemia, multi-dreams, restlessness,abdominal distention after food and deglut and pantothenic acid, the foursymptoms were significant difference between the two groups p<0.05. multi-dreams (P=0.001), restlessness (P=0.0029),abdominal distention after food (P=0.006) and deglut and pantothenic acid (P=0.012), there were no manifest differences in symptoms between first, middle, post in the chemotherapy.Conclusion:1. The means of urinary amylase activity in leukemia and spleen-stomach deficiency group, and the hepatic qi stasis group,all were higher thanthe control group, but the difference is quiet. It indicated that maybe tumor-producing amylase.2. The patient in leukemia group PNA,PSA,ConA-binding ratios of urine amylase were significantly higher than the control group. and the spleen-stomach deficiency group, LCA,PNA,PSA,ConA-binding ratios of urine amylase were significantly higher than the control group, no significant variation in the hepatic qi stasis group. It indicats that amylase sugar chains in both leukemia and spleen-stomach deficiency group have significantchanged, urine amylase core-fucosylated,high-mannose-type,hybrid-typesugar chains increased, terminal sialic acid and fucose on the sugar chain decrease, D-Gal on the sugar chain exposed more. this exist in only children leukemia with spleen-stomach deficiency, not exist in children leukemia with hepatic qi stasis, maybe spleen-deficiency syndrome type play an important part in generating aberrant amylase sugar chains. To considerspecific lectin-binding ratios of urine amylase as one of the reference index for spleen-stomach deficiency diagnosis.3. The ONOO~- of leukemia group and hepatic qi stasis group, and the MDAof leukemia and spleen-stomach deficiency group were both lower than control group, lt is considered that the situation was related to diseases andchemotherapy.4. The patient in both leukemia group and spleen-stomach deficiency group, urinary amylase activity was significantly positive correlation with MDA, but the correlation was not manifest with ONOO~-, in hepatic qi stasis group and control group. It's consider that MDA as one of main factorsthat affecting amylase activity of spleen-stomach deficiency. A positive correlation was found between PNA, PSA, ConA-binding ratiosand ONOO~-.a negative correlation was found between WGA-binding ratios andMDA. It indicated that maybe free radical play an important part in generating aberrant amylase sugar chains.5. Multi-dreams, restlessness, abdominal distention after food and deglut and pantothenic acid, the four symptoms were significant difference between weakness of spleen-stomach deficiency group and hepatic qi stasis group. there were no manifest difference in symtoms between first, middle, post in the chemotherapy. It indicated that four symptoms had clinical sense in indentifying spleen-stomach deficiency and the hepatic qi stasis.
