Background: Acute cerebral infarction is characterized by high morbidity,high disability rate. The treatment for it is far from satisfactory.Theoretically, to timely dissolve the thrombus in cerebral vessel can rescuepart of the brain cell and improve the curative effect on acute cerebralinfarction. Comprehensive treatment contributes a lot to post intra-arterialthrombolysis, but there is no such report. TCM treatment has good effect foracute cerebral infarction. The brain disease center of Guangdong ProvincialTCM Hospital develop intra-arterial thrombolysis combining yin yangSyndrome Differentiation System for acute cerebral infarction, and get thegood clinical effect.Purpose: Initially, the author analyzed the effect and its shortcoming ofintra-arterial theombolytic comprehensive therapy adopted by GuangdongProvincial TCM Hospital, as to provide evidences for further improvement ofintra-arterial theombolytic comprehensive therapy.Methods: This study's style is case analysis analyzing acute cerebralinfarction 22 cases that underwent intra-arterial thrombolysis in GuangdongProvincial TCM Hospital in the period from March, 2005 to February, 2007. Searchemphases are analyzing in yin yang Syndrome Differentiation System Framework.The main evaluating indexes are: 1. Recanalization rate; 2. The cases ofintracranial hemorrhage occurred /percentage; 3. The cases ofmortality/percentage; 4. the percentage of patient has almost recovered. (BI>75,NIHSS≤1, mRS≤1).Result: The general recanalization rate was 75%(18/24 vessels), and thebleeding rate was 22.7%(5/22 cases), among of the 5 cases, there were 4 PH casesand 2 cases died of the 4 cases PH. The mortality within 90 days was 19.0%(4/21 cases, excluding of the patients that failed to be followed up).SIRS scores≥2, 8 cases(8/22), among of which, there were 3 cases diedduring hospitalization. 6 cases' syndrome differentiation were yinsyndrome, and 2 cases' were yang syndrome. The recanalization rate, the bleeding rate, the incidence of PH and thepercentage of NIHSS scores have almost recovered in 21 days, and the percentageof BI,mRS score have almost recovered in 90 days, which indicates that thereis no significant difference between the yin and yang syndrome.Conclusion:(1)The recanalization rate, the bleeding rate, the incidence ofPH and the curative effect on day 21 and day 90, which indicates that thereis no significant difference between the yin and yang syndrome. (2) Contrastwith the datum announced by "Ninth Five" Science and Technology Key Taskon intravenous thrombolysis for acute cerebral infarction within 6 hours, wecan come to the conclusion that the risk of intra-aterial thrombolysis needrigorous experimental evaluation.
