| Anxious depression has anxious and depressive symptoms coexist and interact. Anxiety plays a more important role than depression in the state of anxious depression. Senior citizens have higher risk than youth. Cerebral blood flow and 5-HT receptor have some relationship with the clinical representation. Studies of drug therapies are carrying out though multiple centre random control test and meta analyzes, which have stronger demonstrative intensity and guiding value. Anxious depression, whose etiology exists in the depression of visceral Qi or depression caused by evil influence, along with the mechanism exists in the retardarce of Qi and blood, the abnormal of transmission and translation of viscera, should belongs to the category of "Yu syndrome" in traditional Chinese medicine (TCM). The five set of Yu affect each other, whose pathological position exist in the middle Jiao along with various kinds of symptom. The chief therapeutic principal is to expedite Qi and regulate the middle Jiao. The therapy of the five set of Yu should regulate the visceral Qi while that of the six set of Yu should expel the six set of evil influence. The modern TCM doctors not only reveal the mechanism of "Yu syndrome" though the theory of "cerebric spirit", experimental models and scales, but also treat it by the traditional or selforganized formula. However, there are few researches on TCM therapies to anxious depression.Objective: to study the relationships between syndrome classification of TCM and self-rating anxiety scale (SAS), self-rating anxiety scale (SDS), for the sake of enriching the syndrome classification methods. To study the therapeutic effect of Xiaoyao powder along with TCM nostrum and mental enlightenment in treating anxiousdepression.Method: 48 outpatients who accord the criterion of anxious depression and syndrome classification of TCM were divided into the "Xiaoyao Powder" group and the control group. The former was treated with Xiaoyao Powder combining with TCM nostrum and mental enlightenment while the latter was just treated with TCM nostrum and mental enlightenment without Xiaoyao Powder. The therapeutic effect was evaluated 4 weeks after the therapies, through calculating the total score with Microsoft Excel 2003 and carrying out statistic analysis with SPSS 11.0, applying Spearman correlate, binary logistic regression to test the relationships between syndrome classification of TCM and SAS, SDS. Result: the majority of patients are women and middle age, belong to hepatic depression and low-grade. Vertigo, slip of capabilities and down in self-worth exist in cardiosplenic asthenia more frequently. Irritability, trend of committing suicide exist in hepatic depression more frequently. There are no item can provide introduction to hepatic renal Yin deficiency. Majority of patient have somatic and mental symptom relaxation after therapy, reach a total rate of efficiency of 95.7%. Effect of low-grade patients or "Xiaoyao powder" group is better than that of high-grade patients or control group.Conclusion: outpatients usually complain about somatic symptoms rather than mental problem, leading to the difficulty of diagnosis. Items of SAS and SDS have more relationship with hepatic depression than cardiosplenic asthenia, indicating the important role of hepatic depression in the pathogenesis of anxious depression. Anxious depression is not the disease of one but multiple viscera, Qi, blood and body fluid. No introduction can the items provide to the hepatic renal Yin deficiency, indicating there still exist some shortage in the introduction of syndrome classification with SAS and SDS. Integrated therapy has preferable effect, however, the patients are chiefly low-grade, who have better effect than the high-grades, indicating the therapy still needs to be improved and the effect needs to be promoted. |
