The Clinical Study Of Yinaozengzhi Capsule On Vascular Dementia

Objective: To evaluate the clinical efficacy of Chinese Medicine YiNaoZengZhi Capsule in treatment of patients with vascular dementia(VD) and investigate the changes of cognitive function,activity of daily living,Phlegm-block and Blood-block symptom .To analyze the mechanism of YiNaoZengZhi Capsule on VD and provide a theoretical basis for clinical treatment of VD. All the persons'cognitive impairment were in state of mild or moderate .Changes of laboratory tests, ECG examination and adverse events which possibly happen during therapeutic process should be recorded In order to estimate the clinical safety of YiNaoZengZhi Capsule on VD .Material and Methods : According to the diagnosis standards made by APA DSM-IV and NINDS-AIREN and elimination scales and Chinese medicine symptom standards fifty-two VD cases were selected(thirty-four were males and eighteen were females). They were randomized into group A (therapy group) and group B (control group). group A had twenty-seven cases (twenty-one are males and six were females) and was treated with Chinese herbal medicine YiNaoZengZhi Capsule and Its average age was 61.67±7.00years old. group B had twenty-five cases (thirteen were males and twele were females) and was treated with Co-dergocrine Mesyiate Tablets and Its average age was 63.48±7.42 years old. Both of the groups were taken orally. The duration was 12 weeks. The scales of MMSE and BRS which were used to assess the effects on cognitive function, ability of daily living, and Chinese medicine symptom were recorded respectively at outset and the end of every 4 weeks; general change was rated with scales of Safety issues, including vital signs, laborary tests and ECG examination weremeasured at outset and at the end of 12 weeks.There were no significant difference in the composition of sex ,age, pathogenesis, pathogenetic condition, the degree of education ,previous medical illness and drug history between the two groups,they had comparability.Discussion: Vascular dementia often generate on the basis of stroke and has special clinical characteristics including acute or subacute onset,volatile conditions, ladder-like decline and so on .So far,VD is the only type of dementia which can be treated.andAccording to the severity of the stability VD can be divided into three phases of plateau ,fluctuation and decline. Therefore, the treatment should be extended from the plateau, shortened fluctuation periods and prevented illness declining.Methods that can intervent cerebrovascular disease risk factors , improve cerebral circulation, protect and activate cerebral metabolism had been used in clicinal treatment.Meanwhile,VD is involved with biological,sociological and psychological fields and its treatment is a integrated process which should be contain a psychological behavior and drug combination,treatment and rehabilitation care combining, traditional Chinese and western medicine combining,patients and family community integrated collaborative. Yinaozengzhi capsule can extend life time, improve intelligence, enhance the antioxidant ability of the body to improve the body defense capability by enhancing the body's ability of removing endogenous free radicals and slowing down vivo lipid peroxidation rate and can inhibit platelet aggregation, reduce the plasma viscosity and prevent thrombosis.It can be play a important role in the prevention and treatment of vascular dementia by increasing the content of SOD and reduceing the content of MDA in rat brain tissue.Results:1.Compared curative of two groups the treatment group didn't surpass the control group(P >0.05);2. Both group A and group B had got significantly improvement after therapy compared with before(P<0.05)in MMSE scores.There was no significant difference between the two groups(P>0.05); 3. Both Co-dergocrine Mesyiate Tablets and YiNaoZengZhi Capsule were extremely effective in amending activity of daily living, But there was no significant difference between the two groups(P>0.05);4. All patients'scores of Chinese medicine symptom dropped obviously after therapy compared with before(P<0.05), group A surpassed group B (P<0.05). 5.All patients had no unusual upset and adverse reaction in treatment liver and renal functions were not abnormal after treatment.Conclusions:1. Chinese medicine YiNaoZengZhi capsule and Co- dergocrine Mesyiate Tablets could ameliorate the mild and moderate cognitive impairment of VD patients.They could significantly rise the MMSE scores,drop the scores of activity of daily living and ameliorate Chinese medicine symptom;However, there were no significant differences between their effects. 2. There were significant improvement on daily habits of the VD patients with YiNaoZengZhi capsule and Co-dergocrine Mesyiate Tablets but YiNaoZengZhi capsule surpassed Co-dergocrine Mesyiate;3.The amelioration on Chinese medicine symptom of VD patients who were treated with YiNaoZengZhi capsule was more significant than those treated with Co-dergocrine Mesyiate Tablets;4. During the tweleve weeks'treatment with Chinese medicine YiNaoZengZhi capsule , there were no unusual upset and adverse reaction and no liver and renal functions harm among all patients. Chinese Medicine YiNaoZengZhi Capsule is safety and utility on VD.5.YiNaoZengZhi prescription is composed of many traditional chinese drug which can active blood circulationg and disspate blood stasis . The possible mechanism of YiNaoZengZhi Capsule on VD is reliefing blood vessel spasm to release cerebral ischemia state and reduce the production of active oxygen. YiNaoZengZhi prescription has preventive and therapeutic effect on VD and can ameliorate cognitive function is probly by improving the ability of antioxidase and inhibiting the reaction of lipid peroxidation.
