Objective: To investigate the techniques of the total hip arthroplasty in the treatment of osteoarthritis secondary to acetabular dysplasia (AD) .Methods: Between July 2000 and Octobur 2006, a total of 38 hips in 30 patients with advanced osteoarthritis secondary to AD underwent the total hip arthroplasty. Among the patients, 6 were male and 24 were female, with their ages ranging from 23 to 59 years and an average age of 46 years. The bilateral arthroplasty was performed in 8 patients and the unilateral arthroplasty in 22 patients. The patients mainly suffered from pain and claudication. According to the Hartofilakidis classification, semi—dislocation occurred in 5 hips , low—dislocation in 23 hips , and high—dislocation in 10 hips: and the Harris scores before operation were 56, 36, 29 respectively.Results : All the patients were followed up for 5 months to 6 years(averaged 3.5 years ). The Harris scores after operation were 94,92, 88 respectively. The differences between peroperation and postoperation were significant. 1 patient in which hip meta-dislocation happened.All the patients were pain—free and there was no sign of aseptic loosening and subsidence.Conclusion: The total hip arthroplasty is an effective method for the treatment of osteoarthritis secondary to AD. Before operation. acetabular bone bed should be measured accurately. In operation, a new acetabular bed should be reconstructed currectly. After operation, and active rehahilitation should be carried out. Most of the patients can obtain satisfactory recent clinical result.
