Objective: To describe the histopathological pattern together with characteristic clinical and imaging features of cryptogenic orgnising pneumonia.Patients and methods: One patient with cryptogenic orgnising pneumonia was described and relevant literatures were reviewed.Result: Cryptogenic orgnising pneumonia is relatively rare, defined histopathologically by intra-alveolar buds of granulation tissue, consisting of intermixed myofibroblasts and connective tissue.The etiology of cryptogenic orgnising pneumonia is still unknown, but infection,connective diseases, drugs, radiations,occult aspiration, transplantation rejection and the aspiration of poisonous gas have been postulated. Cough and progressive dyspnea are common manifestations. Imaging espscially high resolution computerized tomography(HRCT) is a very useful examination before operation,and histopathological diagnosis can be made by the examination of the specimen .We can get the specimen through percutaneous lung biopsy, transbronchial lung biopsy, video- assistant thorascopic biopsy and open lung biopsy. Corticosteroid treatment in COP results in rapid clinical improvement and clearing of the opacities on chest imaging without significant sequelae.Relapses are common upon stopping or reduction of corticosteroids,but relapses are not associated with increased mortality nor long term functional morbidity.Conclusion: The diagnosis of COP is usually suggested by clinical and radiological findings, but needs to be confirmed histopathologically. Lung histological appearance obtained during a percutaneous or bronchoscopic procedure appears to be an effective method for diagnosing this disease,if negative can't exclude the diagnosis,then that needs to make diagnosis through video-assistant thorascopic biopsy or open lung biopsy.
