Objective: Cervical cancer is one of the most common malignancy of the female genital tract, and its morbidity and dead rate is the first position in the malignancy of the female. In recent years, the age tends to youthful. Meanwhile,pathology changes. In past, squamous cell carcinoma is more than 90%, adenocarcinoma and nonsquamous less than 10%. The former is 70% and the adenocarcinoma is 26%. The changes and patient's demand to raise living quality cause new problem for cervical cancer treatment .Neoadjuvant chemotherapy(NACT) is a new therapy method discussed in recent years. Its objective is to reduce cancer volume, transfer range and clinical staged for locally advanced cervical cancer (LACC). This creats chance for operation, enhance patients'quality of living.It contains arterial and veined chemotherapy based on way. Arterial infusion chemotherapy can cause drugs come in cancer's artery directly , focus in local, avoid being cleaned firstly. It decreases the combining of drug with protein and prolongs the drug's time. Meanwhile , arterial infusion acts on tumor directly, the dosage circling in tissue is less, thus, whole body's poisoned effect decreases obviously. This article is to discuss the efficiency and clinical significance of iliac arterial infusion emboliztion chemotherapy in patient with cervical cancer.Methods: There are 22 cervical cancer cases in our hospital and the first hospital of ShanXi medical university, from Jun, 2004 to Sept, 2006. In these patients, stage Ib2 is 8 cases, IIa 6 cases, IIb 4 cases, IIIa 2 cases, IIIb 2 cases. They accepted iliac arterial infusion emboliztion chemotherapy(interventional treatment).We used cisplatin (CDDP) 60㎎,camptothecin (HCPT) 20mg,mitomycin (MMC) 16mg,5-fluorouracil(5-Fu) 1.0. The response to chemotherapy was assessed by pelvic examination, ultralsound B and histological analysis.Results:1).Twenty-two cases accepted twenty-five neoadjuvant chemotherapy . nineteen cases accepted one time, three was two times. The interval is two weeks. The clinical remssion rate was 95.45% and overall clinical response rate was 86.36% with 12 cases complete responses and 7 cases partial remession.2). Two cases with parauterine invasion had not a significant reduction underwent radical surgery.3). Twenty cases received radical total hysterectomy, and there were not any cancer cell in the stump. Pelvic lymph node metastasis is 2/20. Other invasion is 1/20. Three patients received radical surgery again after operation because dangerous factors .4). The follow-up time was two years for 22 cases. One case was dead and two cases had a recurrence during this period. The median disease-free survival for 20 was 17 months.Conclusions: Arterial infusion embolization chemotherapy can effectively reduce tumor volume, remise clinic symptom, increase surgery rates, decrease lymph node and subclinical metastasis and postoperative recurrence rate. It can also improve existence period and living quality.
