Objective: To explore the clinical value and outcomes of the treatment for comminuted fractures of proximal humeral by using replacement of humeral head prosthesis, and to find out the suitable method for treating the comminuted fractures of proximal humeral.Methods: Depending on retrospective analysis, we reviewed all cases operated on the comminuted fractures of proximal humeral from January 2002 to December 2005 in our hospital. Forty cases operated on replacement of humeral head prosthesis. Forty cases underwent open reduction plus internal fixation. They were not only in accordance with our selection standards but also were followed up. These cases were divided into two groups. One was humeral head replacement group the other was open reduction plus internal fixation group. We used the Constant Scores to make out the excellence rate of clinical outcome and to compare the different excellence rate of the two groups. Moreover, through statistical comparison of operating time, blood loss and the postoperative motion scores of the shoulder, we evaluated the clinical outcomes and their relative factors.Results: There were 40 cases in the humeral head replacement group were evaluated. According to the Constant Scoring excellent in 16 cases, good in 18 cases, moderate in 3 cases, poor in 3 cases and the excellence rate was 85%.
