Objective To improve the acquaintance of idiopathic dilatation of the puhnonary artery, to decrease the rate of misdiagnosis and missed diagnosis. Methods To report a case confirmed by physical examination, chest radiography, computed tomography, and the result of magnetic resonance. Results Idiopathic dilatation of the puhnonary artery is an uncommon but not rare disease, the etiopathogenisis of this disease is not well known at present. Its main characteristic is the enlargement of the main pulmonary artery, with or without dilatation of the right and left pulmonary or peripheral puhnonary artery. Commonly, patients are asymptomatic, or exercise-induced dyspnea. The diagnosis can be made with the result of chest X-ray, computed tomography and magnetic resonance, but other causes of central puhnonary artery enlargement must be excluded. Conclusions Idiopathic dilatation of the puhnonary artery should be considered when a systolic murmur presents over the left sternal border or a left hilum mass was revealed on chest x-ray. Idiopathic dilatation of the puhnonary artery is benign, commonly need no therapy, but long-time follow up is necessary.
