| Objective: Breast cancer is one of the most common carcinoma in woman, which of the morbidity is 31 percent in carcinoma of woman. Almost all the cause of death were distant metastasis. There are no evidence of metastasis by physical examination or biochemistry and imagine examination when first visit. 30 percent patients would recur or metastasize in 5 years even after a radical operation and no invade of axillary lymph node by pathohistology evidence. It is a key point to find the patients with potential risk of metastasis and give them active treatment. Maspin, a unique serine proteinase inhabitor, has been shown to inhabit tumor cell motility and invasion, Which maybe an important factor to evaluate the metastasis and prognosis for patients of breast cancer. Maspin mRNA expressions in the tissue of breast cancer of 30 patients with primary breast cancer were detected by the reverse transcriptase-polymerase chain reaction (RT-PCR). At the same time human mammaglobin(hMAM) mRNA in peripheral blood of the 30 patients were detected. With these results combining with the pathohistology diagnosis of axillary lymph node, we would analyse the relationship between maspin expression in primary breast carcinoma and micrometastasis and axillary lymph node metastasis.Method: Select 30 patients whose the pathology diagnosis all were infiltrating ductal carcinoma in our hospital. 8 cases of them were found the metastasis of axillary lymph node by pathohistology. All the cases were taken 10ml blood from ulnar vein after anaesthesia and before operation. Isolated the mononuclearcell by add the lymphocyte isolation solution andadd 1ml Trizol solution, after that the samples were stored in the freezer below -20℃.Split the cancer nodus into two pieces, one of them was for pathohistology, the other one was stored in liquid nitrogen after be delt with the phosphate buffer solution. Total RNA were extracted first , then cDNA were synthesized by the template of RNA, we got the amplificated production by PCR at last and observed the results under viltalight lamp after gel electrophoresis. All data were managed by 12.0 SPSS statistics, applied X2 test.RESULT: All the amplificated production of peripheral blood shown a band of 528bp of GAPDH (intro- reference), and some of them shown a band 361bp of hMAM. All the amplificated production of breast cancer tissue shown a band of 294bp of GAPDH(another intro- reference), and some of them shown a band of 129bp of Maspin. 1. In the 8 cases with metastasis of axillary lymph node, 7(87.5%) were negative for expression of Maspin mRNA. In the other 22 cases without metastasis of axillary lymph node, 7(31.8%) were negative. There are significant different between the two groups. 2. in the cases of positive expression of hMAM, 11(73.3%)cases were negative for expression of Maspin mRNA. in the cases of negative expression of hMAM, 3(20%)cases were negative. There are significant different between the two groups.3. In the 8 cases with metastasis of axillary lymph node, 8(100%) were positive for expression of hMAM mRNA. In the other 22 cases without metastasis of axillary lymph node, 7(31.8%) were positive. There are significant different between the two groups.Conclusion: Maspin, as a inhibitor for invasion and migration of tumor, the mRNA expression in tissue of breast cancer shown low level in the patients with metastasis of axillary lymph nodes or micrometastasis of peripheral blood. It can be one of the factors for predicting the metastasis of axillary lymph nodes and micrometastasis and maybe signify unfavourable prognosis. |
