OBJECTIVES: This study using a swine model of chronic end-stage congestive heart failure produced by right ventricular rapid pacing was designed to test the safety and therapeutic effectiveness of self bone marrow mononuclear cells'transplantation.METHODS: Ten swine were equally divided into experimental group (B group) and control group (A group) randomly after right ventricular rapid pacing for four weeks. Bone marrow mononuclear cells (BM-MNCs) were aspirated from self bone marrow which was taken from the iliac crest of swine. The same volume of the BM-MNCs or culture medium was transplanted into the models through the cardiac catheter. In following four weeks both groups had a reduced rate (190bpm) of pacing designed to maintain heart failure. Two-dimensional echo cardio-graphic studies were recorded at the time of baseline, four weeks pacing later and four weeks post-transplantation. At the same time, the change of left ventricle pressure was detected by Powerlab8SP and Millar catheter. We also tested the VEGF and bFGF level in serum after cell transplantation. In the end, swine's hearts were excised to make pathological sections. We observed whether the cells transplanted were surviving in the hearts by the means of immuno-histochemistry or immunofluorescence staining. Vessel number was also counted in experimental group and control group by immuno-histochemistry staining with anti Factor-VIII antibody in experimental group and control group.RESULTS: Four weeks after transplantation, left ventricular function, including ejection fraction (EF), cardiac output(CO),left ventricular systolic pressure (LVSP),±dp/dtmax, et al had a significant improvement, meanwhile left ventricular end-diastolic dimension(LVEDD), left ventricular end-systolic dimension(LVESD) and left ventricular end-diastolic pressure (LVEDP) were decreased in group B.
