| Background : Primary liver cancer is malignant tumor primary at hepatocyte or epithelial cells of bile duct, we can detect the trace of PLC in the ancient literatures of TCM. It belongs to Guzhang(Cirrhosis ascites), Ganji(Liver acumulation), Zhengjia, Jiju, Huangdan(Jaundice), Xietong, etc. It is highly malignant and prognosis is bad. It is showed by the data from the Ministry of Public Health in 1995 that PLC stants44/100, 000 of mortality rate taking the second place among all of the malignant tumors. An earlier period operation is still considered as the best treatment plan. However the majority of the patients of PLC are already in mid and late stage when finally diagnosed or complicated by Cirrhosis of liver and miss their opportunity of resection. Sincel959, 3D-CRT was put forward and developed quickly. In the local therapy, It gain accurate effect to the PLC missing opportunity of resection therapy and be widely used .TCM is becoming one important skill to treat PLC. There are more research of the Treatment on PLC with TCM combining normal radiotherapy, and little research is done on combining with 3D-CRT. It is worthing more forward research on this place to get one better therapeutic method.Objective : Abserve and evaluate the therapeutic effectiveness for PLC by the therapy with TCM combining 3D-CRT to find an effective project to abtain a long survival rate, improving Quality-of-life(QOL)of PLC patients. Methods: Study 30 cases of middle-advanced stage PLC in Guangdong Province Hospital of TCM from January 1998 to September 2005, Who have accepted the radiotherapy given by Fisher three dimensional treatment planning system(3-DTPS) and Varian 600C/D linear accelerator. During and after the radiotherapy all the patient was given Chinese Madicine orally according their symptoms. Using the software of SPSS. 11.0, the curative effects are evaluated according to lump variety, symptoms, Karnofsky performancestatus(KPS), the childs grade of liver function, QOL, Median survival times(MST), and the survival rate etc.Result: The objective therapeutic evaluation of tumor with CR was2 cases, PR, 21cases, NC, 6cases, SD, leases and the total effective rate was 76. 67%;The AFP levels were reduced significantly and there exsists notable statistics sense(P<0. 01);the effective rate in reliving major symptoms was 86.67% and the effective rate in KPS was 73.33%. 17 cases of the patients increased their body weight by the treatment;two case had a survival time above 5 year;1 case , obove five years;lease, obove three years;2 case , above two years;12case , obove 1 year, and the other are less then one year, the everage survival time is 24. 07 (95%CI: 15. 46-32. 68) months, median survival time is 15.00 (95%CI: 10.09-19.91) months. It is better than the simple 3D-CRT. Little harmful effect was appear during the treatment. Cone I us i on : Research showing that: The effect of Chinese Medicine combining 3D-CRT on PLC in middle-advanced stage is better by abserving the change of lump variety, symptoms, Karnofsky performance status, the childs grade of liver function, QOL, Median survival times (MST) and the survival rate etc. It can prolong the patients' survival time, improving Quality-of-life of PLC patients, and decrease the side effect. This can be considered as one effective method of treating PLC. |
