| THE COMPARISON OF THE EFFICACY BETWEEN THREE-DIMENSIONAL CONFORMAL RADIOTHERAPY(3DCRT)ALONE AND 3DCRT PLUS TRANSCATHETER ARTERIAL CHEMOEMBOLIZATION(TACE)FOR TREATMENT OF PRIMARY LIVER CARCINOMAObjective:To evaluate the therapeutic effects of three-dimensional conformal radiation(3DCRT)and 3DCRT combined with transcatheter arterial chemoembolization(TACE)for the unresectable primary liver carcinoma (PLC).Methods:From April 1999 to July 2007,a total of 236 patients with PLC who could not receive surgical therapy,153 patients were treated with 3DCRT alone and 83 patients were treated with TACE plus 3DCRT.In the 3DCRT group,the total radiation dose 33.6~70Gy(median 52Gy),2~8Gy(median 4.6Gy) each time,irradiation 3~5 times every week,the total of irradiation time is 6 30(median 11).In the 3DCRT+TACE group,TACE was performed using lipiodol, cisplatin,perambicin,mitomycin,gemcitabinehydrochloride, hydroxycamptothecin and gelatin sponge cubes.Results:The efficacy rate in short-term for the 3DCRT group and the 3DCRT plus TACE group was 37.3% and 42.2%,respectively.The 1-,2-,3-year survival rate was 53.7%,29.4%and 18.9%,and 65.9%,32.5%and 25.5%,respectively.No significant difference for the efficacy rate or survival rate was found in the two groups(P>0.05).Conclusion:The 3DCRT iS effective and feasible and can be chosen as one of the first—line means in treating unresectable PLC.CLINICAL DOSIMETRY STUDY OF THREE-DIMENSIONAL CONFORMAL RADIOTHERAPY FOR UNRESECTABLE PRIMARY LIVER CARCINOMAObjective:To assess the efficacy of 3-dimensional conformal radiotherapy (3DCRT)of unresectable primary liver carcinoma(PLC)patients with different dose of radiation and fraction.Methods:From April 1999 to July 2007,236 unresectable PLC patients were treated with 3DCRT.They were classified into three groups according to the dose of radiation:there were 49 patient in group BED<66GyGy,143 patients in group BED 66Gy~76Gy and 44 patients in group BED>76Gy.In group BED 66Gy~76Gy,the patients were divided into 3 groups according to the fraction.There were 18 patients in group fraction<401cGy,51 patients in group fraction 401cGy~500cGy and 74 patients in group fraction>500cGy.To compare the effective rate and the overall survival rate of all these three groups.Results:The effective rate for patients in group BED<66GyGy, group BED 66~76Gy and group BED>76Gy were 25.5%,32.1%and 40.9%, respectively.There was no significant difference between the three groups(P>0.05). The 1-,3- and 5-year overall survival rates of group BED<66GyGy,group BED 66Gy~76Gy and group BED>76Gy were 38.1%,18.1%,15.5%,67.5%, 39.5%,26.3%and 51.9%,17.7%,0,respectively.There was significant difference between the three groups(P<0.05).The survival rate of group 66Gy~76Gy was better than other groups.In group BED 66~76Gy,the effective rate for patients with fraction<401cGy,401cGy~500cGy and>500cGy were 16.7%,31.4%and 58.1%,respectively.The short-term efficacy of group fraction>500cGy was better than other groups.The 1-,3-and 5-year overall survival rates of group fraction≤400cG,group fraction 401cGy~500cGy and group fraction>500cGy were 56.3%,0,0,53.8%,31.1%,17.0%and 78.4%,47.3%,34.0%,respectively. There was no significant difference between the three groups (P>0.05).Conelusions:The present results suggest 3DCRT is a practical treatment option in unresectable PLC patients and BED 66~76Gy and fraction>500 cGy and≤700cGy is highly suitable. Background and Objective:To evaluate the therapeutic effect of three-dimensional conformal radiotherapy(3DCRT)on unresectable small primary liver cancer(PLC).Method:From January 2000 to September 2007,38 patients with unresectable small PLC or unwillingly undergo operation were treated with 3DCRT.The mean value of GTV was 56.47±22.46cm3(12.85~95.36 cm3). Radiotherapy was performed at an exposure of 2~8 Gy/time,3~5 times/week, 52±5.6Gy and 6~35(11±3)times.Results:Follow-up all patients showed a complete response(CR)in 18 of 36 patients(47.3%),a partial response(PR)in 15 patients(39.5%),2 patients showed stable disease(SD)(5.3%)and 3 patient showed progressive disease(PD)(7.9%).The overall survival rate at 1,3,5 and 7 years was 94.6%,75.5%,61.9%,and49.5,respectively.Conclusions:The results suggest that,3DCRT is an effective and feasible approach to treat patients with small PLC. |
