Clinical Research On 36 Patients With Aortic Dissection

Objective To investigate the clinical characters of aortic dissection (AD), diagnoses, misdiagnoses reasons and the methods of treatment.Methods The clinical features of 36 patients with AD were studied, including clinical manifestations, diagnoses, misdiagnoses reasons and the methods of treatment.Results (1) Past history: Among 36 patients with AD, 28 cases have hypertension history(78%), 4 cases have Mafan syndrome(11.1%). Symptom: The initial symptoms were bursting pain in 34 cases(94.4%).The location of pain: chest back, 24 cases(67%); epigastrium, 7 cases(19.4%); low back pain, 3 cases(8.3%); chest stuffy panting, 10 cases(27.8%); shock performance, 13 cases(36.1%); headache and dizziness, 6 cases(16.7%); limbs weakness, 4 cases(11.1%); limbs ache, 2 cases(5.6%). Sign: enlargement of cardiac, 7 cases(19.4%); aortic diastolic murmur, 5 cases(13.9%); chest and abdominal vascular murmur, 23 cases(63.9%); limbs artery pulsation weakness or disappearance, 4 cases(11.1%). Complication: aortic incompetence, 5 cases(13.9%); heart failure, 2 cases(5.6%); pericardium effusions, 3 cases(8.3%); pleural effusions, 4 cases(11.1%); renal dysfunction, 2 cases(5.6%); dissection rupture, 3 cases(8.3%); brain block, 1 case(2.8%); leg necrosis, 1 case(2.8%); enteron necrosis, 1 case(2.8%). (2) 20 cases were diagnosed according to CT and TTE, 12 cases diagnosed with MRI and TTE methods, 4 cases diagnosed with CT and TTE methods, according to the combination of the two radiological methods and the symptoms of AD, 36cases were diagnosed, the rate of diagnoses was 100%.
