Objective:To analyze the clinical data of aortic dissection(AD)patients with abdominal pain,and to discuss the clinical features,reasons for misdiagnosis and prevention points.Methods:The clinical data of AD patients admitted to The second hospital of JiLin university from October 2013 to December 2017 were collected.The patients were divided into abdominal pain as the main symptom group(43 cases)and the non-abdominal pain as the main symptom group(219 cases).The clinical data include patients' age,gender,mezzanine parting,laboratory and imaging examination,treatment and prognosis.And the abdominal pain as the main symptoms of AD patients misdiagnosed reasons were analyzed.All data were analyzed by SPSS 19.0 statistical software.Counting data was processed by chi-square test.The measurement data were analyzed by variance analysis and non-parametric test.P values less than 0.05 were considered to denote statistical significance.Results:Among 262 patients,186 cases were male and 76 cases were female.Among them,30 patients with the main symptoms of abdominal pain and 13 female patients.There was no statistically significant difference between the two groups of AD patients with different clinical symptoms(P>0.05).The age of AD patients with different clinical symptoms in the two groups was compared,and the difference was statistically significant(P<0.05).The Stanford B type of AD patients with abdominal pain as the main symptom was more than that of Stanford A,and the difference was statistically significant(P<0.05).The difference between two groups of AD patients with different clinical symptoms was not statistically significant(P>0.05).The difference of the positive result rate of the two groups of AD patients with different clinical symptoms was not statistically significant(P > 0.05).In the treatment of conservative treatment,the effect of abdominal pain type AD patients was better than that of non abdominal pain type AD patients,the difference was statistically significant(P<0.05).In the treatment of surgical treatment,the effect of non abdominal pain type AD patients was better than that of abdominal pain type AD patients,the difference was not statistically significant(P>0.05).In the intervention group,the two were effective,and the difference between the two groups was analyzed,and P>0.05 was found,the difference was not statistically significant.The overall prognosis of the two groups of patients with abdominal pain type AD was better than that of non abdominal pain type AD patients,the difference was not statistically significant(P > 0.05).The misdiagnosis rate of AD patients with abdominal pain as the main symptom is high,and it is easy to be misdiagnosed as digestive system disease.Conclusions:(1)There was no statistical difference in the gender,presence or absence of hypertension and positive correlation between abdominal pain and non-abdominal pain type AD patients.(2)The interlayer type Stanford B type of abdominal pain type AD patients is more than that of Stanford A.(3)There was no significant difference between the overall prognosis and the non abdominal pain type of patients with abdominal pain type AD.(4)The AD patients with abdominal pain as the main symptom are easily misdiagnosed as digestive diseases and need the vigilance of clinicians.
