Background and object: ACI (acute cerebral infarction) or cerebral ischemic stroke refers to irreversible local lesion caused by dysfunction of brain perfusion and successive ischemic-hypoxic necrosis. The incidence of ACI keeps increasing recently. ACI lesion consists of ischemic center and ischemic penumbra. The reversibility of damage to the ischemic penumbra tissue makes a point in treatment and rehabilitation, therefore to protect the reversibly damaged neurons is critical for ACI therapy. Conven-tional early thrombolytic therapy may reduce the infarction volume and restore the neural function that temporarily lost by rescuing the penumbra. However, thrombolytic therapy is very restrictive for stroke patients because of its short RTW (reperfusion time window) and hemorrhagic compication. It may cause more severe reperfusion injury if beyond the RTW. With the progress of molecular biology, recently it is found that the ischemic cascade reaction, called"ischemic cascade", including ischemic-hypoxic energy metabolic dysfuntion,release of excitatory neuromediator, calcium over inflow, free radical reaction, cell apoptosis and so on, makes the central part of ischemic brain injury. According to this theory, interventions of biochemical and metabolic disorder at different steps in the cascade reaction is known as neural protection, it is rather to protect brain cells than to restore blood stream, therefore its indication is wider.
