Objective Compare the difference of normal prostate tissue,BPH and prostatic carcinoma in the expression of VEGF and VIP.So we can investigate the roles and clinical significance of VEGF and VIP in the carcinogenesis.Material and Methods Use light microscope, Verhoff hematoxylin and immunohistochemistry to view the vascular endothelial cells, glandular cells of 10 cases of normal prostate tissue, 15 cases of BPH, 15 cases of prostatic carcinoma. Result (1) HE dying show normal prostate tissue is composed of duct and gland alveolus, and has two layers of cells in its epithelium: inner is the superficial secretory cells and exothecium is the bottom cells which inserting basal membrane and inner epithelium.The bottom cells are smaller and have less kytoplasm. Their nucleus are tenuous and institia, and their polarities are anachromasis and parallel with basal lamina. The structure and epithelial appearance of duct and gland alveolus do not have much difference. The tissue of BPH has much secretory cells and epithelial clusters are visible. Some nucleus are bigger and some are smaller. Some nucleoli and basal cell layers are visible. The secretory cells of prostatic carcinoma have much heteromorphism and their appearance take more multiplicity. They are usually multiplayer or monolayer. The epithelial clusters are not so obvious as normal prostate tissue and BPH and some of them aligned overjetly. The nucleus are bigger, anachromasis and have big nucleoli. Some basal cell disappeared and some cells of glands infiltrate into interstitium. The basal cells of this area disappear, which called micro-infiltration.(2) VEGF are positive in prostatic carcinoma, and are distributed in kytoplasm, matrix cell and fibrocyte, lymphomonocyte and new vascular endothelial cell that aroud the tumor. VEGF are negative in normal prostate tissue and BPH, but are positive in interstitial substance. The intensity of expression of VEGF and microvessel density(MVD):prostatic carcinoma 1.95 ± 0.7.51.1 ± 15.3,BPH 0.9 ± 0. K 27.0 ± 3.1, and normal prostate tissue 0.5 ± 0.K 15.3 ± 2.7. The intensity of expression of VEGF have positive correlation with microvessel density. ( r =0.589,P<0.002)o(3) VIP are ambly-positive in the myenteric nerve plexus of normal prostate tissue,BPH but positive in prostatic carcinoma.The positive granula in normal prostate tissue is 0.66 + 0.2,in BPH is 0.94 ±0.25, and in prostatic carcinoma is 1.87±0.8.The comparison between normal prostate tissue and BPH is significant,and the comparison between BPH and prostatic carcinoma is also significant.Conclusion ( 1) The histology of prostatic carcinoma is different from normal prostate tissue. The duct and gland alveolus deranged and infiltrate. Most prostatic carcinoma are diagnosed by anaplastia of nucleus. The nucleus of cancer cell are bigger than normal cells.(2 )We observe VEGF widespreadly distribute in caner cells, interstitial cells and vascular endothelial cells. The intensity of prostatic carcinoma is larger than normal prostate tissue and BPH(P<0.01),which suggests the expression of VEGF is significant for the development, progress and vascular proliferation of tumor. VEGF can also be a significant indication of determining benign or malignant prostatic carcinoma and metastasis hazard. The MVD of prostatic carcinoma is also larger than normal prostate tissue and BPH(P<0.01)o( 3 ) VIP immunologically competent endocrine cells can reside and express in prostate tissue except stomach, intestine .liver, gall and pancreas. So we can use VEGF and VIP to determine the benign or malignant prostatic carcinoma and metastasis hazard.(4) The expression of VEGF and VIP change from low to high in the process from normal prostate tissue and BPH to prostatic carcinoma.
