Objectives: To explore the functions of 5-hydroxytryptamine(5-HT), nitric oxide(NO) and homocysteine(HCY) in the chronic daily headache(CDH). To discuss the pathogenesis of CDH and to help the diagnosis and treatment.Methods: To investigate 40 outpatients and 20 health persons from Qilu hospital of Shandong University. In all the patients, transformed migraine patients, chronic tension tyel headache patients, new daily persistent headache patients, hemicrania cntinua patients and headache patients from drug abuses were 11, 12, 8, 3 and 6, respectively. Venous blood specimens collected from all the patients and health persons were anticoagulanted, centrifugated and frosted. The contents of 5-HT in plasma and platelets , NO and homocysteine were examinated through spectrophotofluorimetry method, nitric acid reduase method and flurescence method, respectively. ANOVA and Newman-Keuls test of SPSS 10.0 software were used to evaluate the difference among groups.Results: The levels of 5-HT (ng/10~9) in platelets and plasma in transfomedmigraine patients, chronic tension tyle headache patients, new daily persistent headache patients, hemicrania cntanua patients, headache patients from drug abuses and patients in control were 425.87+17.21, 50.64 + 10.12;415.67+15.36, 51.36 +9.38;398.65 + 13.24, 49.89±12.34;379.76±12.69, 48.96±11.36;406.68 + 13.45, 53.65+13.16;789.57+27.12, 80.05 + 14.45 repectively. Each patient group had significant difference compared with the control group(P < 0.05). In addition, the levels of NO (umol/L) in patient groups mentioned above were significantely higher than those in control group too( P < 0.05). The final testing results were 92.76 +19.60, 89.76+18.36, 93.25 + 13.31, 87.36+12.54, 86.56 + 15.46 and 53.88± 12.36. What's more, the levels of homocysteine (umol/L) in patients of various types and health control were 14.24+4.5, 15.36 + 3.3, 13.59 + 3.7, 16.21+4.8, 14.32 + 5.6 and 12.15 ±3.1(P > 0.05), No significant differences were found among the heahdache patients of variou types (P > 0.05).Conclusions: Both the levels of 5-HT in plasma and platelets and the levels of NO in CDH patients displayed difference with those of the health control. It suggested that 5-HT and NO took part in the pathogenesis of CDH. Any treatment to enhance the levels of 5-HT or cut down the levels of NO in the patient bodies might benefit the curement of this disease. On the contrary, the level of homocysteine seemed to have little relationship with CDH.
