The Comparation Of The Effect Of Three Operation Methods Of Treatment For Femoral Intertrochanteric Fracture

Objective: To compare the curative effect sonfemoralintertrochanteric fracture for three kinds of different treatmentmethods: internal fixation of angle blade plate, dynamic hipscrew(DHS) and gamma nail, and make theoretical analysis to offerbasis on choosing the proper treatment method in clinic.Methods: We retrospective reviewed the clinical andradiological records of 208 patients with femoral intertrochantericfracture for three kinds of different treatment with distractiontreatment fracture for three kinds of different treatment withinternal fixation of angle blade plate(angle blade plate group,55cases)or dynamic hip screw(dynamic hip screw group,119 cases angleblade plate group,119 cases) or internal fixation of gamma nail(Gamma nail group,34 cases) from June 1999 to June 2005. Al1these patients were followed-up for 10 months to 6 years with anaverage of 32 months. To compare and analysis the curative effectson femoral intertrochanteric fracture for three kinds of differenttreatment methods from The averages tay, the incidence ofcomplications, the functional evaluation of hip joint (the rate ofexcellent and good results).Resurts : The average stays were 23.39 days for the angle blade plategroup, 21.15 days for the dynamic hip screw group ,and 20.25 daysfor the gamma nail group. There were no significant difference(P>0.05) in these three groups. The incidence of complications were21.82% for the angle blade plate group , 4.20% for the dynamic hipscrew group, and 2.94% for the gamma nail group. There weresignificant difference between the angle blade plate group and theother two groups(P<0.01),and there were no significant differencebetween the dynamic hip screw group and the gamma nail group(P>0.05) . According to the kudema's hip functional standard score,the rate of excellent and good results were 85.45% for the angle bladeplate group, 95.79% for the dynamic hip screw group, and 94.12% forthe gamma nail group. There were significant difference between theangle blade plate group and the other two groups(P<0.01),and therewere no significant difference between the dynamic hip screw groupand the gamma nail group (P>0.05).Conclusion: 1,We can find the high incidence of complications andThe angle blade plate has high incidence of complications and the rateof excellent and good results of the kudema's hip functional standardscore is low, so it is not the first choice for treating femoralintertrochanteric fracture. 2,Dynamic hip screw and gamma nail areboth the best choice for treating femoral intertrochanteric fracture,There were no significant diffrence in these two groups of theincidence of complications and the rate of excellent and good resultsof the kudema's hip functional standard score。...
