PrefaceWHO estimated that 1/4 persons of the world are psychiatric patients, among the totally, half of them are fine diagnosis and only half of them could accept proper treatment. The study of GBD from WHO show that nervous disease and mental disorder is the most heavy burden account 18%. In China, the burden of depression account 6. 9% is second only COPD. In our psychiatric out- patient clinic, many patients have ever seen a doctor in general hospital, not only spend much of charge but also lost the best healing juncture, accordingly they were out of social function even committed suicide when their depression aggravated. In the past, we focus on schizophrenia, when we computed disease burden, even though sometime we thought of depression, we only study in the charge for diagnosis and treatment of depression. In fact, the part of 95% untreated patients is the most important part of the disease burden. To solve the problem, first of all is the correct diagnosis. We will study depression of the third class general hospital medical department and traditional Chinese medicine in Shenyang.ObjectiveDescribe the characteristics and prevalence of mental illness in the outpatient services of different departments in general hospitals in Shenyang. Gain the constituent ratio of any psychiatric disease. Compare theratio of medical clinic and traditional Chinese department. Survey the diagnostic treatment and research condition.Methods 23 general hospitals were randomly selected as research sites. Psychiatric nurses screened 6900 consecutive patients seen in outpatient departments using GHQ. In persons screened positive and 10% of those screened negative, a psychiatrist conducted a structured interview that generated gold -standard diagnoses for different types of mental disorders based on the internationally recognized American "Diagnostic and Statistical Manual of Mental Disorders-Fourth Edition (DSM-IV)".ResultsThe prevalence of any type of mental disorder in general hospital was 16. 77% a higher prevalence in the third general hospital was 19. 06%. In the third general hospital the prevalence of Depressive Disorder was 11. 53%. Patients treated in traditional Chinese medicine department had a higher prevalence than those treated in internal medicine departments. Of the 658 cases of mental disorders identified, their physicians had recognized only 14.ConclusionThere are much of psychiatric patients in the general hospital;especially in the third general hospital and depression is the most important disease. The diagnosis and treatment from doctor is fairly low, most of the patients have not received proper diagnosis and treatment. |
