BackgroundIn the morden world, the development of vehicles is fast, and motor cars play a more and more important role, but the casualties caused by traffic accidents are getting higher and higher than those in the past. There were more than 100,000 people who died for traffic accidents in our country in 2003. It is reported that the most important cause of death of men between 18 and 35 years old is severe head injury in traffic accidents, and the high mortality and disability rates are caused by not only the bain injury but choke and neurogenic pulmonary edema, which cause severe injuries of multiple organs by hypoxia. Recently we have tried to restore the effective ventilation of patients with head injury by transnasal tracheal cannulization in first aids, which improved the success rate of first aids and decreased the mortality and disability. The study evaluated the significance of transnasal tracheal cannulization in the first aids of serious head injury in the traffic accidents. MethodsAll patients enrolled in the study for serious head injury were randomized into three groups, Group A (a reference group), group B (transnasal tracheal cannulization group) and group C (peroral tracheal cannulization group). Glasgow coma scale (GCS), SaO2, mortality, disability rate, incidence of Acute Respiratory Distress Syndrome (ARDS), intracerebral pressure (ICP), the time and cost in hospital were compared in three groups.Results1 .The rates of mortality, disability and ARDS in group B were lower than those in group A, and the time and cost in hospital were less than those in group A. ICP in group B decreasd significantly and was similar with group C.2. The time of the procedure in group B was significantly less than that in group C.ConclusionTransnasal tracheal cannulization in first aid of serious head injury is effective.
