Background and purpose: With the socio-economic and transportation development, traffic injuries are becoming more common, have gradually become the prestige of a rib of human life worldwide pollution in developing countries, the leading cause of injury, first of all traffic accidents, followed by violent conflict. In China, traffic injuries are under the age of 45, the first segment of the population lives in the cause of death and loss of work the first reason. The main reasons for drink-road traffic injuries:(1)Speeding, fatigue driving, take the fast lane; (2)Pedestrian violation, crossing the road or line of motorcycle Road; (3)Roads, vehicle condition and other factors. In severe head injury, caused by the accident accounted for 70.4%, because the brain is the center of human life where the injury prone to respiratory, circulatory disturbance, and asphyxia, respiratory arrest and shock than the other parts of the injury incidence rate of high Therefore, traumatic brain injury in serious condition; therefore, special anatomical features of brain determines the severity of brain injury and its treatment more timely. Severe brain injury is the world's human traffic death and disability, an important reason for disability. The purpose of this subject to explore the risk factors of road traffic accidents and prognostic factors, and for the development of road traffic accident prevention measures to improve the success rate and provide a scientific basis to improve the prognosis.Study design: on the 2008 and 2009 in our hospital, 80 cases of severe head traffic injuries (GCS≤8 points) were retrospectively analyzed to explore the factors associated with prognosis for clinical rescue, first aid targeted to implement effective measures to improve the treatment of severe traumatic brain injury success rate of traffic. According to the Glasgow outcome with GOS as the measure to a better prognosis (good and moderate disability), 30 cases in the control group, with poor prognosis (severe disability, plant survival and death) of the 50 cases for the experimental group. The information collected mobilization software with spss13.0 T test and chi-square test.Result :1. 80 cases included 58 cases of male patients, 22 females, mean age 34.69±18.026 years, and with good prognosis group of 19 male patients and 11 female, mean age 36.47±16.889 years old and poor prognosis group of men 39 patients, 11 female, mean age 33.62±18.76 years.2. Good prognosis group of 30 patients (37.5%) and poor prognosis group, 50 patients (62.5%). Of which there were 72 cases of cerebral contusion (90%), brain stem injury in 29 cases (36.25%), diffuse axonal brain injury in 20 cases (25%), subdural hematoma in 51 cases (63.75%) epidural hematoma in 20 cases (25%), subarachnoid hemorrhage in 49 cases (61.25%), skull fracture in 61 cases (76.25%).3. Good prognosis group of three save time, the average time was 9.90±1.539 minutes, 11.00±1.661 minutes, 80.03±57.624 minutes; poor prognosis group of three forward time, the average time was 11.00±2.474 minutes, 12.92±3.891 minutes, 116.00±83.185 minutes. Three time periods P values were <0.05, that were statistically significant.4. Good prognosis group of transportation: motorcycle 7 cases, 23 cases of vehicle; poor prognosis group of vehicles: motorcycles in 23 cases, 27 cases of car. The two groups P = 0.028 <0.05, statistically significant.5. The study included 80 patients with associated injuries are mainly fractures, lock, rib fractures, pelvic fractures, pulmonary contusion, hemopneumothorax, liver and spleen damage, shock and poor prognosis group's shock rate far greater than the good prognosis group .Conclusion: The results indicate that the prognosis of severe brain injury and traffic accident to the time the ambulance arrived at the scene, ambulance arrival time of the emergency department, emergency department time and are closely related, means of transport for the motorcycle is severe brain traffic injury is a potential risk factor. Eliminate or control these risk factors can change will greatly improve the prognosis of severe brain injury traffic. At the same time, we should strengthen the protection of the vulnerable populations injured, and further strengthen traffic law enforcement, and enhance road safety education.
